&NA;A cross‐sectional design was used to determine whether a relationship exists between the total mileage run each week and the plasma high density lipoprotein cholesterol (HDL‐C) concentration. Ninety of the subjects were experienced runners, whereas the remaining group (N = 19) had done no endurance training. HDL‐C in the runners was significantly correlated with miles run each week (r = .50, P = .001), and this correlation remained significant even when corrected for age (r = .49, P < .001), alcohol consumption (r = .50, P < .001), and percent body fat (r = .40, P < .001). The subjects were then divided into four groups: nonrunners (NR), low‐mileage runners (L, 10 to 19 miles per week), intermediate‐mileage runners (I, 20 to 39 miles per week), and high‐mileage runners (H, 40 or more miles per week). There was a significant, stepwise increase in HDL‐C values from the NR up to the H groups (NR, 34 ± 7 mg/100 ml; L, 47 ± 7 mg/100 ml; I, 53 ± 9 mg/100 ml; and H, 60 ± 9 mg/100 ml; P < .01, and a similar increase in the ratio of total cholesterol to HDL‐C (NR, 5.47 ± 1.3; L, 4.52 ± 1.0; I, 3.99 ± 0.9; H, 3.31 ± 0.7; P < .01). Total cholesterol demonstrated no inner group differences. There were no differences between the groups in diet, alcohol consumption, or parental history of cardiovascular disease; however, the NRs were significantly younger, and among the runners the higher‐mileage groups were significantly leaner (P < .001). These results suggest that HDL‐C concentrations are positively correlated with the average weekly mileage run, even when adjusted for body composition, and are independent of age.

&NA;Thirty‐two asymptomatic men were screened for the presence of coronary disease by stress thallium scintigraphy and upright bicycle radionuclide angiography. Thallium perfusion defects demonstrated a sensitivity superior (92%) to global (85%) or regional (62%) abnormalities detected by radionuclide angiograms, without loss in specificity (95% versus 85% and 95%, respectively). All patients with coronary disease developed at least one abnormal response. Also, the profiles of ejection fraction response to stress revealed an overshoot elevation above resting values in early recovery, even in those with disease. This finding suggests that ejection fraction must be measured at maximal exercise for it to have diagnostic usefulness, especially in those not limited by chest pain.

&NA;The physiologic response and the perception of exercise intensity (RPE) of 41 coronary patients (age 49 ± 7.1) were evaluated after four months of a physiologically ineffective low intensity exercise program and again after four months of a moderately intense program. A nonexercising control group consisted of 13 patients. Training sessions were twice a week for 45 minutes each time and included aerobic exercise and flexibility, muscle strengthening, and breathing exercises. Energy expenditure was estimated at 2.5 kcal/min and 2.5 to 4.5 kcal/min during the low and moderate intensity programs, respectively.The significant changes (P ≤ .01 to .08) that occurred after the low intensity program were decreased heart rate (HR) at similar work loads only in patients suffering from angina pectoris; reduced systolic blood pressure (SBP), only in patients without angina; and decreased SBP × HR (double product) and RPE, in patients both with and without angina.Following the moderately intensive program, double product and RPE decreased further, mainly in the angina group. Comparison between pretraining and post‐moderate training results showed significant changes in most of the parameters measured. No changes were found in the control group. Correlation (r) between double product and RPE was .91 to .95 in the exercise tests.This low‐level exercise program resulted in a training effect, and the perception of exercise intensity was highly correlated with double product, which was consistent with a linear relationship between the subjective response to aerobic exercise and myocardial oxygen consumption.

&NA;Supervised physical training for 12 weeks beginning two months following aneurysmectomy and endocardial resection was assessed in our study. Aerobic consumption improved by 108%; resting and exercise left ventricular ejection fractions increased from 30% to 38% and from 33% to 41%, respectively; cardiac output improved by 23% at rest and increased by 76% from rest to maximum exercise, a 32% rise over baseline value.Thus, exercise training soon after surgery is safe and may prove beneficial as part of the total rehabilitation program for certain patients.

&NA;The purpose of this study was to determine whether alterations in left ventricular dimensions and mass are associated with the increase and maintenance of maximum oxygen uptake (VO2max) with training and reduced training. The exercise consisted of bicycling and running 40 minutes a day six days a week. After 10 weeks, the subjects were divided into two groups that continued to train either four days a week or two days a week for an additional 15 weeks. Intensity and duration for the additional 15 weeks remained the same as for the tenth week of training. VO2max increased approximately 25% (bicycle testing) and 20% (treadmill testing) after ten weeks of training and remained at these levels following 15 weeks of reduced training. Resting heart rates were significantly lower after 10 weeks of training and continued to remain lower after 15 weeks of reduced training either four or two days a week. Echocardiographic measurements of septal and posterior wall thickness in diastole were not changed after training. There was a small but significant trend of increase for left ventricular end‐diastolic dimension (LVID) (4.93 ± 0.16 to 5.14 ± 0.16 cm) and calculated LV mass (76.7 ± 9.0 to 85.9 ± 9.2 gm). After reduced training, LVID returned to control levels; calculated mass also decreased somewhat but still remained significantly above control values. These data suggest that left ventricular responses may be only minimally involved in the processes responsible for the increase and maintenance of aerobic power with training and reduced training.

&NA;To assess left ventricular filling in coronary artery disease (CAD), echocardiography was performed on 50 CAD patients and 20 normal subjects. Posterior displacement of the posterior wall of the aortic root during the first third of passive diastolic filling (D1) and during atrial contraction (D2) were determined. An abnormal D1/D2ratio (normal ≥ 0.87) was found in 35 of 50 CAD patients. A reduced ejection fraction (< 55%) determined by cineangiography occurred in 22.9% of CAD patients with an abnormal D1/D2ratio. Abnormal left ventricular filling is common in CAD patients. Diastolic function of the left ventricle at rest in CAD may be abnormal even when systolic performance is normal.

&NA;Activity and exercise needed to be evaluated and quantitated in patients recovering from myocardial infarction in a study of the effects of an early exercise program on rehabilitation. An activity summary questionnaire was developed to assess the highest level of activity and exercise, week by week, and to record the time of resuming selected activities such as driving, sexual activity, and working. Categories of assessment included indoor and outdoor activities. This questionnaire was used to evaluate 245 patients recuperating from myocardial infarction. A high degree of agreement (>83%) between two independent raters of a representative sample of responses showed the coding of the questionnaire to be highly objective as well as valid. For example, patients who reported walking greater distances showed lower functional impairment (r = ÷ .31, P < .001). With this questionnaire, trends in patient activity can also be recorded during the crucial months after sustaining a myocardial infarction. Such information is of value to health professionals in assisting patients recovering from myocardial infarction with an exercise prescription and counseling about activity.
