摘要:
Coronary heart disease (CHD) is a condition with significant psychologic and social, as well as physical consequences. As such, psychologic assessment is an important component of any cardiac rehabilitation (CR) program. A multidimensional framework is proposed, with particular emphasis on five psychosocial domains: personality function and psychopathology, neuropsychologic function, physical health status stress and type A behavior, and social and work adjustment. The results of the psychologic assessment should emphasize the relative strengths and adaptive resources of the patient, as well as problem areas. Efforts should also he made to integrate psychologic test results with evaluations from other disciplines within the CR team so that a coordinated treatment program can be formulated.

摘要:
The majority of patients with acute cardiac illnesses adjust well psychologically during the in-hospital phase of treatment. Expected reactions to the stress of an acute myocardial infarction (Ml) include anxiety, denial, and despondency. Effective management of all patients admitted to a coronary care unit includes orientation to the environment, anticipation of the patient's fears, opportunity for ventilation, and pharmacologic treatment at anxiety. When effective care is compromised by excessive anxiety, depression, delirium, or behavioral disturbance, psychiatric consultation will be necessary. Patients scheduled to have cardiac surgery require psychologic preparation, and psychiatric attention is advised when the patient exhibits undue preoperative anxiety, depression, or cognitive disturbance. Factors predisposing to delirium following cardiac surgery include preoperative cognitive impairment, history of MI, difficulty of the surgical procedure, severity of recovery room illness, and intensive care unit environment. Management of delirium requires a systematic search for precipitants and predisposing factors, resolution of correctable abnormalities, and use of neuroleptic agents.

摘要:
Psychologic stress has been linked to the development and progression of cardiovasculay disease. Biobehavioral treatment approaches aimed at reducing the impact of environmental stress have been developed as an aid in the reduction of cardiovascular risk. The present review describes the basic techniques of four different biobehavioral treatment approaches popular for cardiovascular disorders. These approaches are; relaxation, biofeedback, anxiety management, and anger management. Available research regarding the application of these techniques to cardiovascular disorders, in particular to hypertension and cardiac arrhythmias, is briefly reviewed. A final summary discusses factors that should be considered when applying these techniques to patients with cardiovascular disorders.

摘要:
The recent resurgence of interest in physical fitness km been accompanied by many claims of associated benefit. The authors review evidence of the acute and sustained psychologic effects of exercise in healthy and in cardiac patients. Research in this field has fawn hampered by methodologic problems; however, many studies suggest that exercise decreases tension and depression while improving self-esteem in subjects who comply fully with training programs. The specific aspects of exercise that produce psychologic benefits have not been determined, hut may include social participation, sense of mastery and achievement, and expectation of improvement.
