Abstract:Pseudohspoxemia or spurious hypoxemia represents a low oxygen tension or saturation in an arterial blood sample that does not correspond with the actual levels in the body. Though rare, its hallmarks include the absence of clinical evidence of tissue hypoxia and normal arterial oxygen saturation measured by pulse oximetry. The causes of this phenomenon include hyperleukocytosis and thrombocythemia. A young patient with leukemia and extreme leukocytosis (hyperleukoctosis) who demonstrated pseudohypoxemia is described herein. Clinical experiments on the arterial blood sample from this patient revealed that the phenomenon of pseudohypoxemia was most likely caused by a rapid in vitro oxygen consumption by the excessively large number of immature leukemic cells. The clinically confounding phenomenon of pseudohypoxemia was also shown to be easily prevented by drastically reducing the time interval between the drawing of the arterial blood sample and its processing by the blood gas analyzer machine.

Abstract:It is sometimes impossible to use conventional forceps to select the bronchus leading to a lesion. We have developed flexible forceps to increase the diagnostic rate of lung tumors at sites here biopsy has been difficult. The flexible forceps are 2.7 mm in diameter and a site approximately 2 cm from the tip can be bent at an angle of approximately 70° by manual rotation of a bolt. This new type of forceps proved useful in three patients in whom biopsy was impossible with conventional rigid forceps.

Abstract:This report describes a rare case of roentgenographically occult small-cell lung cancer (SCLC) diagnosed in a 67-year-old man after resection of locally advanced lung squamous cell carcinoma of the left lower lobe. One month postoperatively, chest roentgenogram and computed tomography (CT) scan revealed no abnormality but bronchoscopic examination showed a dome-shaped lesion with engorged capillaries on the spur between the posterior and anterior segmental bronchi of the right upper lobe; a biopsy confirmed the diagnosis of SCLC. The patient received a combination of chemotherapy and radiotherapy. Bronchoscopy performed after this therapy showed no evidence of tumor and he attained a complete response. One year and 8 months after the initiation of therapy for SCLC, he died of recurrent brain and liver metastasis. The histologic type of this recurrence was not determined. However, this recurrence seemed to be caused by SCLC because of the markedly elevated level of Pro-GRP (gastrin-releasing peptide), which is highly specific for SCLC.

Abstract:The diagnosis of obliterative bronchiolitis (OB) in lung transplant recipients is difficult to establish. We report a case in which this diagnosis was suspected clinically although both bronchoscopic lung biopsy (BLB) and high-resolution computed tomography (HRCT) scanning were inconclusive. The diagnosis was strongly suggested during bronchography through the flexible bronchoscope using iotrolan (Schering AG. Berlin. Germany ). a newly developed, water-soluble contrast medium.

Abstract:Persistent bronchopleural fistulas are associated with a high mortality rate and present difficult challenges for the chest physician. Initial management includes pleural space drainage with chest tube thoracostomy and intravenous antibiotic therapy. Although adequate drainage and control of infection are important aspects of effective therapy, these interventions are frequently not curative. Subsequent treatment will vary depending on the magnitude and duration of air leak, the underlying cause. and the patient's overall medical condition. Surgical closure of a persistent fistula coupled with pleural space obliteration is frequently considered for patients who are good surgical candidates, and successful fistula closure using bronchoscopic techniques has been reported over the past 20 years. Although this treatment modality has been used primarily in patients who are poor surgical candidates, bronchoscopic therapy has also been used as an adjunct to surgery or as sole therapy. This article discusses the role of bronchoscopic therapy in management of bronchopleural fistulas, and reviews the techniques, outcomes, and complications that have been reported in the English-based literature since 1970.
