&NA;In conventional bronchoscopy, detection and localization of dysplasia and carcinoma in situ have been limited to roughly 30% of the total number believed to be present. Using bronchial epithelial autofluorescence in response to blue light illumination, these lesions are detected more readily. Results obtained with laser‐induced autofluorescence (LIFE‐Lung‐System) achieve an increase in the detection rate from 1.3 to 2.7 times that of conventional bronchoscopy. Typical artifacts have to be distinguished from true lesions, and certain benign changes interfere with autofluorescence. This lowers specificity and necessitates more biopsies. However, there are no complications associated specifically with this examination. It can be performed during routine bronchoscopy without the aid of photosensitizers, and examination time is prolonged by approximately 10 minutes. Newly developed systems using filtered light may prove to be equally effective, but data so far are limited. Improved ability to diagnose preneoplasias and early lung cancers will not only result in more lives being saved but will also provide new insights into carcinogenesis.
