摘要:
The solutions to the problems of medical malpractice and malpractice insurance coverage are primarily in the hands of state lesislatures. Lesislation enacted has been primarily palliative, to assure continued availability of professional liability insurance. Unfortunately, no limit can be placed on the costs of such coverage. Unfortunately, too, no long-term solution has been forthcoming. Any long-term solution must encompass some method or methods of reducing injuries to patients and at the same time changing the system from defense of the physician to compensation of the patient. If such changes are not forthcoming, physicians will become uninsurable and the private practice of medicine as we now know it will disappear in this country.

摘要:
One of the disadvantages of conventional circle absorber breathing circuits is that twisting between tile inspiratory and expiratory limbs sometimes occurs. In situations where access to the patient's airway is limited, such as during neurosurgical and head and neck operations, this disadvantage may lead to accidental disconnection or extubation. Another is that the temperature and humidity of inspired gas are usually less than desirable, predisposing to heal loss.1and drying of tracheal mucusa.2We have constructed a circuit that houses the inspiratory limb entirely within the lumen of a slightly larger expiratory limb. This makes it easy to handle and allows heat exchange between the cold inspired and warmer expired streams. Heat of condensation liberated by condensation of water on the outer surface of the inspiratory limb enhances vaporization of water droplets normally deposited on the inner surface of that limb, thereby increasing inspired humidity.

Neurologic complications are uncommon but potential hazards of epidural anesthesia. Of the various causes, epidural abscess is rare; in fact, only one case has been reported previously.1This is the first documented report of paraparesis due to an epidural abscess following epidural analgesia.

Treacher-Collins syndrome, a first-arch congenital defect, causes severe facial deformity.1The syndrome is caused by a fetal vascular anomaly depriving the first visceral arch of its blood supply, mainly from the stapedial artery, between the third and fifth weeks of gestation.2First-arch syndromes may occur in varying severity, some so slight as to he unnoticed. The disease was first described by Treacher-Collins3in 1900 and later described by Francesehetti and Klein.4It is characterized by antimongoloid obliquity of the palpebral fissure; notched lower eyelids; coloboma; mtcrophthalmia; hypoplasia of malar and mandibular bones; large mouth with irregular teeth and inalocclusion; deformed external, and sometimes middle and inner, ears. Associated with these characteristics may be deafness; preauricular blind fistula; dwarfism; cardiac defects; and occasional skeletal defects.5

摘要:
Iunovar, a popular intravenous anesthetic, consists of a short-acting narcotic (fentanyl) and a long-acting tranquilizer (droperidol). It is associated with a number of undesirable side-effects, including respiratory depression, prolonged somnolence, and disorientation.1–3Respiratory depression is easily reversed with nalorphine or naloxone; however, there is no known antagonist for the somnolence and disorientation. Physostigmine is effective in reversing anticholinergic poisoning1–9and phenothiazine-induced coma.10It has been suggested that physostigmine might have nonspecific analeptic actions in the central nervous system and thus be of value as an antagonist to other CXS depressants.10This study was undertaken to evaluate physostigmine as an antagonist to somnolence and disorientation after Innovar-nitrous oxide anesthesia.

摘要:
Although large doses (0.5–3.0 mg/kg, iv) of morphine plus oxygen have little effect on cardiovascular dynamics in critically ill patients undergoing open-heart or other major operative procedures, incomplete amnesia and awareness are not infrequent complications of this technique.1Use of nitrous oxide as a supplement during morphine anesthesia prevents awareness but results in significant cardiovascular depression.2,3Diazepam and droperidol have also been employed as supplements during morphine anesthesia to prevent awareness! However, the cardiovascular effects of these drugs after large doses of morphine have not been investigated. This study was undertaken to determine the hemodynamic effects of two 5-mg doses of diazepam or two 2.5-mg doses of droperidol in receiving 1.5–2.3 mg/kg morphine for open heart or major vascular operations.
