Quantitaion of actual and expected complete compensation for chronic hypercapnia in man is facilitated by addition of data of Brackett et al. in the form of an are to the Siggaard-Andersen alignment nomogram. Perecntage of compensation is defined as 100 times the ratio of observed to predicted (fully compensated) base excess at the patient's Pco, Extracellular fluid base excess, defined as BE3is estimated from a 3-g hemoglobin line added to the nomogram.

Hepatic injury following balothane anesthesia is rare,1,2whereas hypersensitivity and biotransformation to reactive products,6,7which may bind to hepatic microsomes, have been considered as cansative factors, no report condiders decreased blood flow in the hepatic artery. This commnication reports thecases of two patients in whom dramatic documented decreases in hepatic arterial blood How occurred during balothane anesthesia. Hepatitis developed in one patient.

The self-inflatable spobgy-cuffed endotracheal tube (Bivona tube) has been used in cases in which prolonged endotracheal intubation is necessary. The tube is most commonly used for prolonged mechanical ventilation in our ingtensive care unit, where the following complication occurred with its use.

Percutaneous catheterization of the internal jugular vein for central venous pressure (CVP) monitoring is used routinely in the operating room.1Of the complications attending internal jugular cannulation, tension pneumothorax during general anesthesia is probably one of the most serious.2Tension pneumothorax associated with internal jugular cannulation has not been previously reported.

Complications associated with the use of nasogastric tubes are, for the most part, infrequent, and mostly self-limited, such as slight epistaxis and abrasion of the pharyngeal mucosa upon insertion. Isolated instances of more serious complications have included laryngeal obstruction, ulceration and stricture of the esophagus, and perforation of the stomach or intestine.1Laryngeal injury, although usually associated with prolonged nasogastric intubation, has also been reported to occur after intervals as short as four to six days.2Presented in a case of severed laryngeal edema with airway obstruction which began 24 hours after passage of a nasogastric tube.

Enflurane (2-chloro-1, 1,2-trifluoroethyl-difluoromethyl ether: Ethrane, Ohio Medical Products) has rapidly become very popular, and is the inhalation agent most frequently used at this hospital. Enflurane is biotransformed in part to inorganic fluorid ion.1Fluoride ion-induced nephrotoxicity has been established as a cause of the vasopressin-resistant polyuric renal failure occasionally seen following exposure ot significant doses of methoxyflurane.2Postanesthetic renal failure in a patient who received enflurane led to measurement of serum fluoride in a search for a possible etiologic factor.

Subsequent to the introduction of etidocaine by Adams in 1972.1a number of well-controlled clincial studies were reported.2–1Clinical comparisons were then made between etidocaine anf other local anesthetic agents currently in use.5–7Most recently, studies in animals8and man9,10have investigated the toxicity and physiologic side effects of thise drugs. Some of those preliminary studies raised questions regarding the relative poteneies of two cocentrattions of etidocaine. 1.0 and 1.5 per cent.5and also the relative effieacies of bupivacaine. 0.75 per cent, and etidocaine. 1.0 per cent, in providing satisfactory “visceral” anesthesia for lumbar epidural amesthesia.7This study represents a controlled, prospective double-blind study of etidocaine, 1.0 and 1.5 per cent, and bupivacaine, 0.75 per cent, comparing the clinical variables of sensory, motor, and “viseeral” anesthesia for lumbar epidural anesthesia for pelvic surgery.

Lorazepam, a new benzodiazepine, has recently been under investigation as a parenterally administered premedicants. These studies, in man, have shown not only that subjects experienced sedation and hypnosis, but also that many experienced a marked lack of recall.In a previous study, we were able to demonstrate significant impairment of human performance, specifically hand-eye coordination, produced by oral administration of Δ9tetra-hydrocamabiinol.2Others have shown the visual tracking task to be sensitie to the effects of drugs such as alcohol and smoked marijmana cigarettes.3–7Therefore, this investigation was done to study the effects of lorazepam and pentobarbital on visual tracking and to determine whether they affected it. We believed this would be important because there is evidence that performance on cirtical tracking tasks correlates with skills used in driving, flying, boating and industrial work.
