Mechanical ventilation of newborn infants has been employed in many centers. Because methods for selection of infants and technique and pattern of ventilation have varied, comparisons of results are difficult. This report reviews the history of mechanical ventilation of the neonate and describes a scoring system developed for the selection of newborn infants for intermittent positive-pressure ventilation (IPPV/I). The clinical diagnosis and management of 204 neonates prior to selection for mechanical ventilation are outlined. All infants were hypercapnic, hypoxie, acidotic and/or apneic prior to initiation of IPP/I. Survival of these infants following mechanical ventilation is discussed in relation to the scoring system used for selection for mechanical ventilation.

Five years' experience with mechanical ventilation of 204 neonates with similar degrees of respiratory failure is described. The survival of these infants is discussed in relation to clinical diagnosis, hospital of birth, birth weight and age at initiation of ventilation. Complications encountered during mechanical ventilation of the infants are discussed. The results of the overall experience suggest that intermittent positive-pressure ventilation (IPPV/I) improves the rate of survival of infants with respiratory failure, and that complications of mechanical ventilation need not be excessive.

In chronically prepared dogs the maximum acceleration of ejected left ventricular blood (Qmax) and the tension—-time index (TTI) were used as correlates of left ventricular myocardial contractility and integrated wall tension, respectively. These variables were measured in the conscious animal and at end-expired air concentrations of 1, 1.5, and 2 per cent halothane and 20, 25, 30, and 35 per cent cyclopropane. Qmaxwas depressed 65 per cent by 1.5 per cent halothane and 33 per cent by 30 per cent cyclopropane. TTI was decreased 29 per cent by halothane and increased 47 per cent by cyclopropane. Cyclopropane decreased myocardial contractility more than has been previously reported in studies in which this variable was evaluated from the relationship between ventricular stroke work and filling pressure. The divergent effects of halothane and cyclopropane on TTI suggest that left ventricular oxygen consumption may be greater during the administration of cyclopropane.

The cardiorespiratory effects of intermittent (IPPB) and constant positive-pressure breathing (CPPB), with and without expiratory resistance, were studied in ten healthy anesthetized dogs. Tidal volume, flow rate and changes in functional residual capacity (FRC) were measured by capacitance respirometry, a new remote monitoring technique which measures intrapulmonary volume changes as a function of changes in surface area. CPPB with and without expiratory resistance siginificantly reduced intrapulmonary shounting and increased FRC, but produced no change in A-aDo2Reductions in cardiac output which occurred with CPPB were directly related to mean airway pressure. CPPB without expiratory resistance but with an end-expiratory plateau of positive pressoure minimized increases in mean airway pressure and secomed to be the safer and better method of CPPB.
