摘要:
The association of renal tubular dysfunction and methoxyflurane appears to be a distinct entity. Evidence relating methoxyflurane and renal dysfunction is clearly inadequate either to condemn its continued use or to remove it from suspicion. Prospective studies to find out whether there is merely a simple dose–response relationship and to determine the importance of the administration of preanesthctic medication, nitrous oxide, and relaxants with methoxyflurane must be encouraged. Until the results of such studies have been evaluated, the Committee on Anesthesia of the National Research Council urges that this agent should continue to be available in clinical practice.

Continuous positive-pressure breathing (CPPB) with an end-expiratory pressure of 10 cm H2O decreased cardiac output and urinary output, with an increase in plasma antidiuretic hormone (ADH). Similar results were obtained after bilateral cervical vagotomy. Oliguria during CPPB was caused neither by ADH release controlled by an intrathoracic volume receptor mechanism nor by changes in PaCO2, but was apparently related to the decrease in cardiac output Arterial oxygenation did not change during the periods of CPPB, but was increased upon release of the end-expiratory pressure. These changes in blood gases can be explained by the measured decrease in cardiac output or by an increase in pulmonary vascular shunting.

The influence of epinephrine on the cardiovascular effects of high (T5) peridural analgesia was studied in 19 human volunteers, each of whom received two injections. In 12, the effects of lidocaine alone were compared with those of lidocaine–epinephrine. Peridural analgesia with lidocaine alone produced small (5 to 10 per cent) changes in cardiac output (CO), total peripheral resistance (TPR) and mean arterial pressure (MAP). During peridural analgesia with Iidocaine–epinephrine a 49 per cent increase in CO and a 37 per cent decrease in TPR resulted in a 10 per cent decrease in MAP. All of these changes from control were statistically significant and were significantly greater than those produced by lidocaine alone. In another seven subjects, the effects of peridural injection of lidocaine–epinephrine were compared with those of injection of saline–epinephrine. The two solutions produced similar effects on the heart, but lidocaine–epinephrine solution caused a significantly greater decrease in TPR, and consequently in MAP, and a greater increase in leg blood flow than did saline–epinephrine solution.

Diazepam (Valium) proved to be effective in preventing local anesthetic-induced convulsions. In cats with recording electrodes permanently implanted in the brain, the median intravenous convulsant dose of lidocaine (CD50) was 8.4 mg/kg. One hour after pretreatment with 0.25 mg/kg of intramuscular diazepam, the lidocaine CD50doubled to 16.8 mg/kg. This dose of diazepam had no discernible effect on behavior or EEG. However, larger doses of diazepam (0.33 and 0.5 mg/kg) caused ataxia and shifted the EEG to slow high-voltage patterns—without appreciably improving protection against seizures.
