The antagonistic effect ofN-allylnormorphine toward the respiratory depression produced by morphine in man was quantitatively recorded and studied.Even in the absence of hypoxia, concentrations of carbon dioxide in the blood during morphine depression were demonstrated which consistently were in excess of those demonstrated after the respiratory depression produced by morpbine had been antagonized by the administration ofN-allylnormorphine.Correlations of respiratory activity with arterial carbon dioxide content indicated that, although the respiratory responses to carbon dioxide were similar both during morphine depression and after its antagonism byN-allylnormorphine, the arterial carbon dioxide content was consistently higher for corresponding degrees of respiratory activity before than after the administration ofN-allylnormorphine.Evidence supporting the theory thatN-allylnormorphine antagonizes morphine depression by the mechanism of drug competition and the displacement of morphine from certain receptors was discussed, and it was suggested that by such a mechanismN-allylnormorpnine restores the respiratory center to a more normal state of sensitivity to carbon dioxide and thus increases respiratory activity when given during morphine narcosis.Occasional indications of increased vasomotor activity and of analeptic effect following the administration ofN-allynormorphine in the presence of morphine depression also were observed and considered compatible with the suggested mechanism of action ofN-allylnormorphine.

摘要:
The levo isomer of 3-hydroxy-N-allylmorphinan tartrate (levallorphan tartrate) is an effective antagonist to the respiratory depression produced by levo dromoran tartrate, morphine and demerol. When administered prior to these drugs, it protects against respiratory depression.The findings suggest that although respiratory depression is abolished, some action of the opiate drugs used persists which enables them to serve as an effective supplement to nitrous oxide anesthesia. Additional observations are in progress relating to this matter.

Five local anesthetics have been testedin vivofor the rate of penetration on the sciatic nerve of cats as measured by the amplitude of the gastrocnemius twitch stimulated electrically at regular intervals.In equiactive concentrations procaine, lidocaine and ravocaine® had essentially the same rate of penetration.Dibucaine was the least penetrant and WIN 3766 the most penetrant of the compounds tested.Recovery from anesthetic blockade was fairly rapid for procaine, lidocaine and ravocaine®, and much slower for WIN 376G and dibucaine. The order of recovery was as follows: procaine > lidocaine > ravocaine® > WIN 3766 > dibucaine.

Present concepts of pediatric preanesthetic preparation are described.Factors involved consist of a modified psychological preparation, a technique of preanesthetic hypnosis produced by a small dose of thiopental sodium administered rectally, procaine amide to diminish reflex hyperactivity, and the use of banthine® as a drying agent.

摘要:
Antihistaminic drags have topical anesthetic properties which have not yet been fully developed. We have employed one of these agents, pyribenzamine in 4 per cent solution, as a topical anesthetic agent for bronchoscopy in 12 cases. There were 2 untoward reactions thought to be due to toxic effects of the drug. Because of an unpleasant burning sensation, increased salivation and inability to obtain consistently an adequate anesthesia, the 4 per cent solution of pyribenzamine was found to be unsatisfactory as employed by us. Further investigation is proposed.

Threshold intrathecal concentrations of procaine produce anesthesia through a selective blockade of the dorsal root ganglions.The occurrence of procaine in threshold concentrations in the spinal fluid of man during peridural anesthesia can account, in part, for the production of sensory block with this technique.The spread of watery radiopaque solution within the peridural space was studied.

A method adapting the mass spectrometer to the analysis of the nitrous oxide content of blood is described. The method involves extraction of a sample of gas from the sample of blood by equilibration at a constant temperature with a gas containing a known amount of argon (in this case, air). The resultant mixture of gases may then be analyzed on the mass spectrometer and the original nitrous oxide content of the blood calculated. This process may be carried out in the presence of varying amounts of the usual respiratory gases and ether vapor. It is shown that simultaneous determinations of the distribution ratio of nitrous oxide and ether in the same sample of blood yield results in close agreement with determinations for these two agents made separately.Accuracy of analysis for nitrous oxide in blood containing a known and substantial amount is shown to be to the order of ± 2 per cent.

Electrocardiographs determinations were obtained on 76 patients who were intubated during anesthesia with thiopental sodium combined with a muscle relaxant. Ninety per cent of these disturbances (or in 60 per cent of the 76 cases) were sinus tachycardia.In 7 per cent the arrhythmias consisted of occasional ventricular premature contractions (2 cases) nodal rhythm (1 case), ventricular tachycardia (1 case) and bigeminal ventricular tachycardia (1 case). Hypoxia was the chief factor in the production of these arrhythmias. It is suggested, therefore, to compensate with proper ventilation for the decreased pulmonary exchange produced by the intravenous administration of thiopental sodium.

The frequency and severity of vagal cardiovascular reactions were studied in 202 patients undergoing chest operations and 88 patients being treated for medical diseases. Four groups of patients were studied. Group I received only the routine preoperative medication of morphine, scopolamine and seconal®. Group II received digitalis in addition. Group III received methantheline bromide, and group IV received digitalis and methantheline combined. The frequency of the vagal reaction among the entire group was highest among those not receiving digitalis or methantheline bromide and lowest among those receiving both of these drugs. Usually, the reaction occurred thirty-two minutes after intubation. Studies on the influence of methantheline bromide upon the medical patients with increased vagal tone due to digitalis intoxication indicate that the former is a powerful antagonist to the vagotonic effect of digitalis. Thus the combined use of these drugs makes possible the utilization of the advantages of digitalis without the fear of increasing vagal reactions which has been for many years a contraindication to the use of digitalis in the preoperative preparation of patients for operation.

摘要:
An attempt has been made to standardize the nomenclature of inhalation systems and techniques. Its justification is the inconsistencies of the past, teaching difficulties under present conditions, and a need for provisions for classifying future methods. A nomenclature providing an insight into the features of different systems is offered and examples are presented. It is hoped that its use will afford a more rational consideration of those factors which influence the choice of a method for the administration of inhalation anesthesia.
