摘要:
The effects of thialbarbital, diethyl ether, and propanidid on the midbrain reticular formation and the cerebral cortex were studied in the cat. Thialbarbital caused marked depression of the activity of the reticular formation and only slight depression of cortical activity. Inhalation of ether caused early depression of synaptic input to the reticular formation without significantly influencing the functional state of the reticular formation itself. In the cortex ether caused early and marked depression. Propanidid caused less inhibition of the reticular formation than did thialbarbital. The significance of the enhancement of cortical responses in the mechanism of the production of general anesthesia by propanidid is not yet clear. The data suggest that there is no universal mechanism of general anesthesia which can explain the effects of different general anesthetics on various structures of the central nervous system.

Hepatic halothane extraction was studied in seven miniature swine. During halothane anesthesia, nylon catheters with Silastic tips were implanted in the aorta, portal vein and common hepatic vein. After three to five weeks halothane was given. An alveolar concentration of 1–1.3 per cent was maintained for an hour; thereafter, the concentration was decreased. The halothane fraction removed from hepatic blood flow was calculated from blood halothane concentrations. From these values it was determined that alveolar concentrations below 0.0026 per cent resulted in nearly complete removal of halothane by the liver, but no significant removal was detected at anesthetic concentrations. The fraction removed was defined as halothane metabolized. Data from this study indicate that results of tracer studies of total body uptake cannot be extrapolated to the anesthetic state. Tracer studies may cause two-to fivefold overestimates of uptake during maintenance of anesthesia, compared with investigations conducted during anesthesia. Similarly, studies of hepatic metabolism during anesthesia grossly underestimate hepatic biotransformation and production of metabolites at subanesthetic halothane levels.

Isolated cat papillary muscles driven at a rate of 12 beats/min at 37.5 C were exposed tojeoncen-trations of cyclopropane, diethyl ether, Ethrane, halothanc, and methoxyflurane similar to those required to produce general anesthesia mvivo.Each anesthetic depressed peak developed ten-sion, maximal dp/dt, and the force-time integral of the twitch, and each shortened the time to peak tension. These variables were altered in qualitatively similar ways by all anesthetics tested, implying a common mode of action on the con-tractile process. When administered in cquieffec-tivc concentrations from the standpoint of producing general anesthesia (i.eat equal MAC's), the order of activity of the anesthetics in depressing contractility (from most to least depressant) was: Ethrane > halothanc > methoxyflurane > cyclopropane > diethyl ether.

摘要:
The effect of intravenous succinylcholine on scrum potassium was studied in 21 combat casualties receiving their first anesthesia within three hours of sustaining massive muscle trauma. In 20 of these patients, there were no statistically or physiologically significant increases in serum potassium. In one patient, an increase in serum potassium was apparent ten minutes after injection of succinylcholine, progressing to a peak level of 6.2 mEq/1, 20 minutes after injection. It is uncertain whether succinylcholine caused the hyperkalemia in this patient.

摘要:
The effects of Innovar on hemodynamics and blood gases were studied in IS adult patients with advanced cardiac disease during analgesia and sedation for angiocardiography. No significant changes in cardiac index or rate, stroke volume, stroke work, or left ventricular systolic and diastolic pressures were observed- Femoral systolic pressure decreased, on the average, from 152 to 139 mm Hg; femoral diastolic pressure, from 74 to 67 mm Hg; mean femoral pressure, from 100 to 91 mm Hg. Systemic vascular resistance decreased from 40.4 to 35.8 units, while pulmonary resistance did not change. Paco2increased from 35 to 40 mm Hg, whereas Paco2decreased from 76 to 57 mm Hg. All these changes were significant.
