The physical act of surgery interested him without troubling him, but he was not able so easily to resolve the moral and ethical questions raised when one person, even a doctor, presumes to cut into another human body and to remove parts of it. The effects of anaesthesia troubled him almost more than the effrontery of human surgery: to induce sleep is to cause the soul to leave the body. This is a serious thing, not to be lightly regarded or undertaken. The competence of the anaesthetist becomes then of more moment than that of the surgeon, for he who has the power to make the soul leave the body for a time must have the power to cause it to return before death results from soul loss. Metabolism and elimination of (d-tubocurarine-H3were studied in 31 dogs. Combined renal and hepatic elimination accounted for over 85 per cent of the injected dose within 24 hours. In the absence of renal function (ligation of renal pedicles), the liver greatly increased its capacity to transportd-tubocurarine into the bile. A search for metabolites ofd-tubocurarine in urine and bile indicated that no metabolites were present in urine, but that approximately 1 per cent of the injected tritium appeared in the bile in alternate molecular form.

The anti-arrhythmic action of dehydrobenz-peridol (DBP) (droperidol) was studied during cyclopropane anesthesia in nine normal female patients during elective surgical operations. Cardiac rhythm was monitored by means of electrocardiography. Ventricular arrhythmias were produced by the intravenous infusion of epinephrine at a low, constant rate. The rate just sufficient to initiate an arrhythmia was defined as the “threshold.” The principal finding was that DBP in a dose of 0.2 mg./kg. doubled the arrhythmic threshold.

Sacs made of inverted frog skin were filled with an electrolyte solution and exposed in a pressure chamber to nitrogen or nitrous oxide with a small concentration of oxygen. Changes in sodium ion concentration were measured after 20 to 22 hours. With increasing concentrations of nitrous oxide in the range of 100 to 200 psig. inhibition of sodium transport increased. Inhibition of sodium transport was shown not to be due to pressureper sesince 175 psig. nitrogen did not result in inhibition. Inhibition of sodium transport paralleled previous reports of nitrous oxide depression of nerve excitability.

The neuromuscular effects ofd-tubocurarine, edrophonium and neostigmine were determined in patients anesthetized with nitrous oxide plus trichlorethylene, meperedine, or thiamylal. The effect ofd-tubocurarine (0.1 mg./kg.) was variable, ranging from no effect on twitch tension to abolition of the twitch response. Because of this variation in response the routine use of a fixed mg./kg. dose ofd-tubocurarine was not recommended. Edrophonium (10-100 mg.) was found to be unsatisfactory as an antagonist ofd-tubocurorine. Neostigmine (2.5–5 mg.), however, was found to be a good antagonist ofd-tubocurarine. Although there is still some controversy concerning the mechanism of anticurare action of edrophonium and neostigmine, cholinesterase inhibition appears to be a major factor.

Algesimetric studies were carried out in volunteers during the inhalation of subanesthetic concentrations of methoxyflurane and halothane. Blood levels of agents were measured in venous samples obtained immediately after each determination of pain threshold. Significant increases in threshold were seen with both agents but a linear relationship between pain threshold and blood levels was not seen consistently with either agent. Significant reductions in threshold were seen occasionally with both methoxyflurane and halothane and were most probably related to the onset of drowsiness. The occurrence of analgesia with low blood levels in the majority of subjects lends support to the current practice of reinforcing nitrous oxide oxygen anesthesia with low concentrations of halothane or methoxyflurane.
