With the increasing use of cocaine in the U.S. population, women of childbearing age have particularly shown an increasing prevalence of use. In an ongoing study at the Perinatal Center for Chemical Dependence (PCCD) at Northwestern University Medical School, 70 infants delivered to cocaine-using women were evaluated. Pregnancy and neonatal outcome were compared to a group of drug-free controls who had no history of evidence of licit or illicit drug use. These controls were selected on the basis of social, demographic, and environmental backgrounds and were comparable for cigarette use during pregnancy. The cocaine-addicted women had a high incidence of pregnancy complications, and infants born to cocaine-using women demonstrated an increased rate of intrauterine growth retardation, prematurity, microcephaly, and perinatal morbidity. Further research will begin. to focus not only on the effects of cocaine per se, but also on the interactive effects of polydrug use, the dynamics of maternal/infant interaction in the substance abusing mother, and the environmental factors that place cocaine exposed infants at high risk for future medical and developmental disabilities.

摘要:
Genetic counseling for parents of children with a genetic disorder is only the beginning of needed support services. One important member of the health team is the genetic support group. Within a genetic support organization, individuals and families discover that they are not alone and that they can be helped by others affected by similar genetic diagnoses. Ideally, a strong partnership between the support group members and interested health professionals will enable the organization to meet its goals. Networks, such as the Alliance of Genetic Support Groups, can help member organizations become a unified force without taking away their individual agendas.

Efforts to prevent perinatal mortality and morbidity are traditionally directed at the pregnant woman. It is during the prenatal period that the mother's health status is closely monitored; her exposure to substances known to be harmful to the fetus is assessed, and intensive patient education on behaviors likely to benefit the unborn child is offered. Unfortunately, the initiation of prenatal care may already be too late to prevent spontaneous abortions, congenital anomalies, and some causes of low birthweight. Until routine prepregnancy care is available to all women of childbearing age, many opportunities for the primary prevention of poor reproductive outcomes will be lost. This paper describes the rationale for prepregnancy or preconceptional counseling and the specific purposes it should serve. A model program providing such services is described, and findings for a low socioeconomic population involved in the model program are given.

Physical abuse of women is pervasive with one in three women experiencing battering. Battering occurs during pregnancy with women reporting blows to the pregnant abdomen, injuries to the breast and genitals, and sexual assault. A recent study in a large metropolitan area documented one in twelve pregnant women to have been physically battered during the present regnany. Among the women battered, 87% had been physically abused prior to pregnancy and 29% reported the abuse increased after becoming pregnant. Health care providers did not assess any of the women for abuse. When pregnancy outcome was analyzed in another study of 589 postpartum women, battered women were four times more likely to deliver a low-birthweight infant. This paper discusses the etiology of battering, prevalence of battering during pregnancy and pregnancy outcomes of battered women. A community-wide primary prevention program is presented that has linked the health care provider with law enforcement and shelters for battered women to interrupt the cycle of violence and promote the health and safety of pregnant women.

摘要:
Each year, more than 250,000 American babies are born prematurely or with a low birthweight. Every two minutes a baby is born with a birth defect. These conditions cause and contribute to more than 40,000 infant deaths annually. Early and regular prenatal care is proven effective in reducing the risk of low birthweight. Given the importance of prenatal care, the March of Dimes advocates the availability of maternity care for all pregnant women. This article outlines the March of Dimes public affairs program, its involvement in the successful expansion of Medicaid coverage for poor pregnant women and infants, and the opportunity for continued advocacy to improve maternal and child health care.
