Historians have usually described contributions of medicine to birth as if all the benefits accrued to patients. This article exolores the contributions that birth and birthing women have made toihe professionalization of American medicineland to the social and economic status of doctors during the past 200 years. Birth provided doctors with some of their first valid techniques, giving them a claim to superiority over irregular practitioners and midwives; it also enabled them to become moral authorities and personal confidantes to women. Later it helped to accustom families to the use. of hospitals. Finally, maternal and neonatal mortality rates provided an international standard for judging the overall quality of a nation's health care. In the United States, high maternal mortality rates led to the first self-critical investigations by the profession, the first hospital mortality review boards, and the first major specialty hoard, obstetrics. In the process of gaining these professional advantages in an entrepreneurial climate, American doctors transformed birth from a natural event into an occasion requiring the use of medical art.

Data from birth certificates were used to examine the pattern of obstetrical procedures used in the care of a sample of low- risk women. The data suggests that hospitals ,with greater technological capabilities and more specialized medical manpower have a greater tendency to use the obstetrical procedures of cesarean section, forceps delivery and drug induced labor. However, the utilization rates of these procedures also varied considerably be- tween hospitals with comparable technology and medical staffs.

The effectiveness of a family-centered maternity pro- gram in promoting a positive feeling toward the infant and in prepar- ing the mothers for their new role was evaluated. Seventy-three married primiparous women completed measures of anxiety and depression as well as a series of questionnaires relating to their at- titudes toward pregnancy. the baby, and the hospital experience. Statistical analyses revealed that the women in the familycentered maternity program. when compared with those in the traditional pro- gram, had a more positive attitude toward their babies while in the hospital. In addition, they reported having received more experience in caring for their babies and more help from the hospital personnel in preparing for the mothering role.

Seventy college women aged 18 to 22 and 67 older women aged 30 to 45 completed a questionnaire comprised of multi- ple choice and open-ended questions covering recollections of menarche, preparation for menstruation, menstrual symptoms, and current attitude. The hypothesis that menarche was a significant developmental event that would be readily recalled by a majority of women regardless of age was confirmed. Almost all of the women remembered their first menstruation and the early experiences described by both groups were similar. Most women felt they were adequately prepared and a significant relationship was found be- tween adequacy of preparation and positive reaction to menarche among the college women. Both groups reported a similar incidence of menstrual pain, bleeding, and amenorrhea. However, the in- cidence of menstrual symptoms in adult women was not significantly related to preparation for menstruation or to whether or not men- arche was a positive event.
