摘要:
Lifting the curse of menstruation is examined as a process of freeing the imagination rather than rejecting the importance of biology. The defining features of the feminist and biomedical perspectives are discussed and an overview is offered of unexamined assumptions about women, the cycle, and the conduct of research which operate within modem scientific medicine. The real alternative to these unexamined assumptions is seen as the development of a revolutionary system of classification in which even the smallest unit of analysis is a multidimensional process. Suggestions are offered about the proper role of the feminist perspective in helping to free our imaginations and move us toward such an alternative paradigm.

Menarche represents a developmental milestone in a woman's life. This paper reviews current knowledge about the physiological aspects of menarche and its place in the sequence of pubertal development. Hypotheses regarding the mechanisms that trigger menarche are presented, as is our current understanding of the influence of hormones, genetic factors, nutrition, exercise, and illness. Also discussed are the ways in which the changes of puberty and menarche affect the adolescent girl's psychosocial development, the unique problems of the early maturing girl, and the kind of preparation for menarche that is needed.

This review article examines factors affecting normal variations in menstrual flow, methods used to estimate vaginal blood loss, menstrual health problems related to hygiene practices, and methods of assessing tampon absorbancy. Preliminary studies from this laboratory suggest that normal, cycling women exhibit menstrual flow rates that are significantly higher during the day than at night; also that menstrual blood has a more acid pH than previously reported, and that it demonstrates unique biological properties unlike peripheral venous blood. Women in wheelchairs may be at special risk for the development of menstrual health problems because of difficulties with hygiene management as evidenced by results of a study of 22 women with traumatic spinal cord injuries.

Studies of the effect of the menstrual cycle on standardized cognitive tasks, work and academic performance, perceptual- motor performance, and psychophysiological measures are reviewed. The weight of the evidence argues against a menstrual cycle effect on behavior. Studies of self report and of behaviors reflecting self confidence suggest that beliefs of menstrual debilitation remain in the population. Studies of atypical and deviant groups indicate a possible connection between behavior and the menstrual cycle.

Pain during the menstrual period is of two types: primary, in which the pelvic organs are normal and secondary, in which pathologic lesions are found on pelvic examination or laparoscopic examination of the pelvic organs. Since the incidence varies between 31-92% of the population it has serious implications for our society. It is important to distinguish dysmenorrhea from the premenstrual tension syndrome. Currently primary dysmenorrhea is thought to be due to increased contractility of the myometrium or decreased uterine blood flow from the excessive contractions or increased sensitization of pain fibers to mechanical and chemical stimuli. These three mechanisms of pain production are due to the release of certain prostoglandins from the endometrium during menses which then go directly into the myometrium producing these effects. Situational or psychological factors may accentuate or decrease the pain. Treatment is with antiprostoglandin drugs. There are several causes of secondary dysmenorrhea both congenital and acquired. Laparoscopy is frequently needed along with D&C to make the diagnosis. After diagnosis is made appropriate treatment can be undertaken to relieve the pain.

This review considers premenstrual syndrome (PMS) from the point of view of definitional issues, treatment issues, and the impact of the recent popularization of the syndrome. With respect to definition, problems include the lack of a consensus among researchers and clinicians concerning number, combination, and variety of symptoms, time cause, criteria for patient/subject selection, relationship of psychiatric disorder(s) to PMS, and determination of the existence of several PMS syndromes vs. one syndrome. Given the problems with definition, recommendations about appropriate treatment are difficult. Most treatment trials have lacked adequate controls for evaluation of placebo effects. The popularization of PMS but the mass media has raised collective consciousness about the syndrome but may also have raised hopes prematurely for a "cure."

Higher rates of mental ill-health, particularly mood disorder, have been reported in women compared to men and this discrepancy has been attributed to underlying biological changes associated with the menstrual cycle. The literature relating to psychiatric ill-health and the premenstrual syndrome is, accordingly, critically evaluated and the evidence favouring a causal role for menstrual cycle changes in the genesis of such ill-health is briefly reviewed. A simple model of causation attributing psychiatric-illhealth to hormonal variation in the menstrual cycle is eschewed in favour of a multifactorial model which assumes an interaction between hormonal and hormonally-related changes in the premenstruum, basic personality and social dissatisfactions, adverse life circumstances, and interpersonal stresses.

摘要:
This paper discusses questions frequently asked by menopausal women regarding body and affective changes associated with the menopause. Menopause as disease is contrasted with menopause as normal. Common menopausal changes and ways in which to deal with them are presented. Answers to some of the questions and concerns that women have cannot be found in the research done to date because of lack of unbiased research on the normalcy of menopause. Thus, this paper illustrates the need for woman-centered research to answer women-related phenomena.
