摘要:
This paper explores male and female obstetricians' attitudes towards their patients in childbirth. Individual interviews were conducted with 50 obstetricians usin a semi structured questionnaire. Across gender and level-oftraining there were few differences in descriptions of easy patients. The few differences that did emerge reveal that attending physicians see patients as less difficult to handle than do residents and that female residents place more emphasis on compliance and control than do male residents. The results of the study are discussed in terms of sex-role stereotypes, the medical school socialization process and the medical model of health care.

Knowledge gained from a literature summary was tested against data obtained from a national volunteer sample of 1,000 battered women recruited primarily through solicitation by an advertisement in Woman's Day magazine. In comparison with other help-sources (social service agencies, the clergy, the police, lawyers, women's groups, shelters), medical personnel were found to have been used fairly frequently, but they were seen by the battered women as less effective than any other group. Quotes taken from indepth interviewers with 146 of the battered wives' are used to bring life to the comparative statistics. Specific reasons for the low effectiveness ratings of the medical profession are considered, and ways to improve service delivery ratings are discussed.

This longitudinal research examined postponement of needed health care by unmarried older women. This research is timely since the unmarried are projected to be an increasing proportion of the aged population. Relationships between being unmarried (widowed, separatedtdivorced. never married) and postponement of treatment, reasons for postponement of care, and changes in health care behavior were examined. Data were analyzed from 1,159 widowed, 264 never married, and 251 separatedldivorced women age 58 to 63 years at the time of the initial interview. Marital status was associated with postponing care and reasons for delaying care. The proportion of women who failed to obtain care and their reasons for doing so were fairly stable over a decade. Distress over finances tended to be more salient in determining postponement of care than health status. Formerly married and never-married women had somewhat different reasons for delaying care suggesting the need for targeted interventions by professionals.

More than half of all mothers with children under age 6 are in the labor force. Working mothers must take off anywhere from 5.6 days to 28.8 days per employee per year to care for their sick children. In a survey of 134 working mothers with children in day care centers. 70% expressed an interest in sick child care options outside the home. especially a sick room at the child's regular day care center or an infirmary at the parent's workplace. Mothers who chose "out-of home care" were more likely to: be minority (p<0.01); be single parents (p = 0.06); earn less than $10,000 annually (p = 0.03); want their children with temperatures of 100-100.9 F to remain in school until the end of the day (p<0.01). Communities and day care centers serving especially lower income. minority or single-parent working mothers should consider investigating these out-of-home sick child care options; the savings to employers could be $2 to $12 billion per year, not to speak of the personal savings to the mothers themselves

Concern about the apparently shrinking role of the British midwife led in 1976 to the formation of the Association of Radical Midwives (ARM). This paper describes thc emergence and ideas of ARM. Comparison of ARM philosophy with the beliefs of a sample of English midwives reveals considerable support for ARM's non-medicalized philosophy of childbirth and its specific proposals for systemic change. Few English midwives, however, consider themselves "radical midwives" or agree with ARM that their role has shrunk and training suffered. The conclusions suggest several reasons why English midwives' support for ARM and its beliefs is limited.

Though there are distinct technical advantages to medical advances allowing prenatal diagnosis of fetal abnormalities, the psychological impact of the use of these procedures has been given little consideration. While early detection of impairment may provide parents with an opportunity for emotional preparation, it also may precipitate a long period of distress. Toward the end of clarifying this indetermmate impact of prenatal diagnosis of fetal abnormality, the literature addressing the emotional consequences of giving birth to an impaired infant is examined for its relevance to the prenatal diagnostic situation. The clinical and research implications of this extrapolation of an existing literature to an important new question are considered. It is concluded that health care pro- fessionals must begin to explore systematically the psychological impact of the use of this technology, and must use their increased understanding clinically to promote not only the physical health of the neonate, but also its emotional wellbeing and that of its parents.

Restrictive regulations ("protective legislation") on employment conditions of female workers limiting maximum hours of work and prohibiting certain toxic exposures have existed for decades. In some countries, such as the United States, Canada and the Nordic countries, the growth of civil rights and equal opportunity legislation has led to their elimination, either in fact or in practice, and only a small number of disparate regulations for male and female workers still exist. Most other industrialized countries, as well as the International Labour Office of the United Nations, still have active restrictive rules for women's employment. However, restrictive regulation is an area of active policy debate around the world. International examples of the debate on protective legislation are given here. A specific case study of the occupational health standards governing exposure to ionizing radiation is used and its technical rationale discussed as an illustration of the basic issues. These include: overbroad categorization of all women as potential childbearers, no matter what their childbearing intentions; failure to recognize the full range of potential adverse health effects to males; disparate application of the restrictive regulations, generally to occupations or areas of employment that are traditionally held by men, while traditional female jobs with the same exposures are excluded from the regulatory restriction.
