摘要:
Poverty results in both personal and structural deprivation for women. Health status and health care are intimately affected by this deprivation As a result, the increasing rates of poverty among women have serious immediate and long-term consequences for their overall health status. Feminist and social structural analyses are crucial. to a comprehensive understanding of poor women's health. Critical to these perspectives is the premise that the underlying structure of American society has produced a social stratification system that has grossly unequalized life chances for poor women. This inequity deleteriously affects poor women's health behaviors, health outcomes, and the health care delivery services that are accessible to them. An adequate understanding of poor women's health calls for an in-depth examination of health and behavior and the social contextual framework within which they occur, taking into account poor women's position in society, the communities in which they live, and the stressful life events they experience.

摘要:
There are approximately 10 million women in the United States who are currently taking oral contraceptives on a daily basis. Although the actual number is not known, it is also estimated that a large number of these same women are on concomitant drug therapy. In recent years there have been a number of published reports linking a loss of contraceptive efficacy with the concurrent administration of other drugs, including antibiotics. Whereas the actual risk of drug interaction between antibiotics and oral contraceptives is yet unknown, it is important that women who are on oral contraceptive therapy be aware of possible contraceptive failure. This article review: the published incidence of oral contraceptive/antibiotic interaction. alone with a discussion of the possible mechanisms by which this occurs. recommendations are also presented for the health management of women taking oral contraceptives and other prescribed drugs.

摘要:
This paper examines obstetricians' explanations for the longstanding and current conflict between obstetricians and nurse-midwives in the District of Columbia over the struggle for independent practice for nurse-midwives. Based on extended interviews with obstetricians, this study shows they are reluctant to provide nurse-midwives with medical backup, a requirement of D.C. law for nurse-midwives to deliver babies in District hospitals. Obstetricians' concerns involve the boundaries of professional responsibility, and economic constraints. Obstetricians believe their more extensive training better qualifies them to handle difficult pregnancies, and they are concerned that nurse-midwives may not call for their help when it is needed. They argue that expanding the use of nurse-midwives will lead to a competition for healthy clients, a competition for paying clients, increased exposure to malpractice liability for obstetricians providing backup, and difficulty obtaining reimbursement from nurse-midwives for their services.

This study reviewed 1,050 articles published in the New England Journal of Medicine, the American Journal of Psychiatry, and the Journal of Consulting and Clinical Psychology in 1982 and 1991/92. The NEJM included the fewest female subjects at both assessments (26.6% in 1982 and 36.5% in 1991/92), and one way analysis of variance showed a significantly smaller percentage of women in the NWM compared to either the AJP or the JCCP (F = (2,1048) = 11.5, p<.001) in 1991192. The NEJM did increase the percentage of women participants over the decazde (~(534) = 3.0, p = .001), but there was no increase in the proportion of its studies including women. Attempts to encourage the inclusion of women in health-related research have been only modestly successful. Medical research, in particular, continues to underrepresent women in its published studies.

摘要:
The American states have broad discretion in developing their respective abortion policies. This paper examines the determinants of 1990 state policies toward funding abortion for low income women and analyzes differences in the outputs of these state policies. Our findings show that the strength of advocacy groups as well as the political forces within a state determine the type of abortion funding policy. Once the policy is established, the number of publicly funded abortions is determined by the type of state policy adopted as well as the demand for these services. The results of this analysis should be informative for the future of abortion politics in the American states.
