Acetabular fractures, although relatively rare, are often serious injuries requiring complex treatment. Major associated injuries are common and the mortality rate continues to approach 20% (8).We retrospectively evaluated 38 acetabular fractures. Twenty-six fractures were treated operatively, with adequate followup available for 23. Anatomic reduction or reduction to within 3 mm was achieved operatively in 21 (81%). The average Harris hip score was 90. The five poorly reduced fractures had an average Harris hip score of 54. Twelve patients were treated nonoperatively with adequate followup available for nine. Six patients with nondisplaced or minimally displaced fracture treated nonoperatively had an average hip score of 96. Six patients had displaced fractures which failed to reduce with nonoperative treatment. Their average hip score was 59.This study reaffirms the importance of adequate fracture reduction in the treatment of acetabular injuries.

During a 3-month period an autopsy study was performed on cases of penetration lung trauma to assess the frequency and significance of air embolism. The technique of detecting air in the chambers of the heart and the coronary vessels was, in the early part of the study, to flood the pericardium with water and observe for air while stabbing the heart chambers. Later it was found preferable to manipulate the removed heart in a basin under water. Twelve cases demonstrated injury to the pulmonary parenchyma alone with no other injury, either intra- or extra-thoracic, to account for death. In nine of these cases significant amounts of air were demonstrated in the heart and coronary vessels using our autopsy techniques. Case reports are summarized to emphasize the clinical picture (unexplained hypovolemic shock, cerebral symptoms) and autopsy findings (evidence of air embolism).Successful management requires an awareness of the condition, correction of those factors exacerbating air embolism, and prompt thoracotomy in order to clamp the injured lung pedicle in patients failing to respond to therapy.

A case of severe bone and soft-tissue injury of the anterior side of the lower leg is presented. Extent of loss of the tibia was 20 cm. Loss of tibia was covered with free vascularized fibular graft extended to 26 cm, which appears to be the longest ever reported. Repair was successful.

The os trigonum is an accessory bone of the foot found in 7% of the normal adult population. It is located at the posterolateral projection of the talus, and can occasionally give rise to symptoms of acute and chronic unexplained ankle pain.We report three patients, one with acute fracture and two with chronic ankle symptoms. Technetium 99 methylene diphosphonate showed intense focal uptake at the posterior talus pointing to the os trigonum as the site of symptoms. It was excised in two patients with complete relief. The third went on to develop an asymptomatic nonunion.We recommend bone scanning as a procedure that is helpful in delineating obscure pain in the ankle that may be due to chronic irritative nonunion of the os trigonum.

A 19-year-old girl presenting with an isolated unilateral adrenal hemorrhage after a motorcycle cross-country race without evidence of direct trauma, is reported. The lesion was diagnosed by abdominal ultrasonography, and treated surgically by thoracolaparotomy. Recovery has resulted. A review of literature shows the rarity of the condition.

A 62-year-old man sustained a barium chloride burn after his jackhammer penetrated a pocket of molten barium chloride. He subsequently developed signs of systemic barium poisoning including bigeminy and hypokalemia. Treatment consisted of early thorough debridement and intravenous potassium supplementation. Serum barium levels were determined. In a review of the literature these appeared to be the first toxic barium levels to be reported. The literature concerning barium poisoning and its complications is reviewed and related to the clinical presentation and treatment of this chemically burned patient.
