One hundred thirty-seven fractures of the scapula in 121 patients were reviewed. The average age at the time of injury was 35 years, with fractures predominant in males (64%). The majority of cases (43%) involved fractures of the body of the scapula, with fractures of the scapular neck being second most common (26%). Automobile accidents produced the most injuries (52%), followed by auto-pedestrian (18%), falls (12%), and motorcycle accidents (11%). Associated bony or major soft-tissue trauma was present in all but 15 of the 121 patients, the most common being fractured ribs in 44%. Other associated injuries included fractures of the clavicle (26%), fractured skull (24%), cerebral contusion (20%), neurologic deficit (13%), and pulmonary contusion or hemopneumothorax (16%). Patients with injuries involving the acromion process or acromioclavicular joint had a significantly higher incidence of associated peripheral nerve injuries than those with other fractures of the scapula, indicating that special attention should be given to the neurologic examination of patients with these fractures. Careful neurovascular examination is mandatory. Treatment was usually conservative, independent of the location of the fracture, with satisfactory long-term results. We recommend simple immobilization followed by early active range of motion exercise.

Abdominal aorta ligation in rats was found to be a suitable model traumatic injury for the study of the effect of trauma on drug-metabolizing enzymes. This procedure was found to result in a rapid decrease in both cytochrome P-450 and mixed function amine oxidase activities. Recovery of mixed function amine oxidase activity was slower than recovery of cytochrome P-450 activity.

We present a report of a 28-year-old male patient who developed a left ventricular to right atrial communication associated with tricuspid insufficiency secondary to nonpenetrating cardiac trauma. The patient's heart block was clarified by serial observation of electrocardiograms and the use of HIS bundle electrocardiography. The patient's fistula was surgically repaired, and 20 months postinjury he is asymptomatic and pacemaker dependent. Echocardiography may aid in the early diagnosis of a left to right shunt and/or tricuspid insufficiency in the setting of possible cardiac trauma.

A case of traumatic disruption of the manubriosternal joint in the absence of rib fractures has been seen. The manubrium was displaced posteriorly on the sternum. Treatment by observation only was followed by a satisfactory outcome with new bone being laid down to produce a false joint.

Four cases in professional athletes and a ballet dancer of what we would like to term the anterior impingement syndrome of the ankle are described. A brief review of the very sparse literature is presented. The cause is felt to be the repeated pull of the anterior ankle joint capsule and the impingement of the talus against the tibia in running and jumping leading to calcific deposits along the lines of the capsular fibers. We feel that any athlete who has pain or limitation of motion secondary to the anterior impingement syndrome should be considered for surgical removal of the offending spurs. The four patients were able to return to their previous activity levels.

Blunt injury to the neck can cause hemorrhage and pseudoaneurysm formation. A 76-year-old patient with severe blunt injury to the anterior neck was found to have entrapment of the left vertebral artery at C2-C3. Arch aortography 3 weeks postinjury revealed a large aneurysm at the injury site. The mass was surgically reduced, followed by ligation of the artery 9 days later, which resulted in prompt elimination of the mass and its accompanying bruit. Rapid recognition of this injury depends on early carotid and vertebral artery angiography.

Exsanguination is the immediate problem when dealing with trauma to pelvic blood vessels. A case is presented to demonstrate a systematic approach for successful management. Embolization, arteriography, and externally controlled compression stopped the life-threatening hemorrhage.
