Two hundred two mortalities from blunt trauma from the Coroner's Office of Ontario were reviewed and compared to 102 mortalities seen at the Regional Trauma Unit at Sunnybrook Medical Centre, Toronto, and its 36 referring hospitals (EMSS). Nonpreventable deaths due to head injury were defined and excluded from review. This left 103 mortalities from the Coroner's Office and 52 from the EMSS. The average postmortem I.S.S. of patients dying in the Trauma Unit was 53, compared to 33 in patients treated outside the EMSS. Of EMSS patients 5.8% had an I.S.S. below their LD50, with 53% below their LD50in the reference group. This postmortem review shows the advantages of an organized system of trauma management.

Post-traumatic diabetes insipidus is a rare entity, usually associated with severe head trauma, often with skull fracture and cranial nerve dysfunction. A 24-year-old male presented to a medical clinic 2 weeks after a minor head injury with presenting symptoms of polyuria with nocturia and intense polydipsia. A diagnosis of post-traumatic diabetes insipidus was made. Although other causes of polyuria syndromes must be ruled out, plotting urine versus plasma osmolalities can provide rapid preliminary diagnostic help when considering diabetes insipidus. First-line treatment should include a trial of chlorpropamide and hydrochlorothiazide. Caution should be exercised in not overlooking associated anterior pituitary damage, which may develop several years post-trauma.

Posterior dislocation of the hip may be complicated by femoral head fracture at a higher incidence than previously described. Three examples of this fracture deformity are presented. These illustrate the use of computerized axial tomography to identify the position of the fracture fragment and aid in planning the therapeutic approach.

摘要:
Posterior sternoclavicular joint dislocations are thought to be exceedingly uncommon. Two recent cases are discussed in terms of diagnosis, management, and potential risk to the patient. One dislocation was successfully reduced with lidocaine infiltrated and the second, seen 72 hours postinjury, required closed reduction under general anesthesia. Soon after injury these patients have a hollow at the sternoclavicular junction. Sternal oblique X-rays and tomograms are helpful in diagnosis. This entity may be promptly recognized and treated in the emergency department with gratifying results.

Awake nasotracheal intubation in the patient with a fractured mandible may be facilitated by combining bilateral superior laryngeal nerve block with topical application of local anesthetic to the nose, mouth, and trachea. Successful use of this technique is described in two such patients.
