The ability of macrophages to phagocytize particulate matter is largely dependent on fibronectin, a nonspecific opsonin found in plasma. Fibronectin depletion, resulting in reticuloendothelial system (RES) depression, occurs following a variety of physical insults. RES depression may contribute to postinjury sequelae such as respiratory distress syndrome and septicemia. Fibronectin concentration was measured in the plasma of sheep with chronic lung lymph fistulas subjected to controlled thermal injury. Fibronectin levels were significantly (p <0.05) decreased at 4, 24, 48, and 72 hours following burning. Fibronectin concentration decreased in parallel with serum albumin concentration; serum globulin concentrations did not decrease. Fibronectin concentration had an inverse relationship to lung lymph flow, a reflection of pulmonary transvascular fluid filtration rate. The role of fibronectin in the pathogenesis of postinjury respiratory distress syndrome deserves further exploration.

Lipoprotein changes have been investigated in 16 patients with burn injury. Serum, urine, and blister fluid were examined for apoprotein content. In the group of patients with moderate burns serum High Density Lipoprotein (HDL) fell drastically, and Low Density Lipoprotein (LDL) decreased slightly, whilst Very Low Density Lipoprotein (VLDL) rose. In blister fluid there was a greater loss of HDL apoprotein than LDL apoprotein or apoprotein C, whilst in concentrated urine a small quantity of HDL apoprotein could be detected. The loss of HDL, which is necessary as an apoprotein C receptor in the conversion of VLDL → LDL, may lead to a block in the conversion of VLDL to LDL. This may in part contribute to the hypertriglyceridemia, hypocholesterolemia, and hypophospholipidemia noted after burn injury.

Twelve adults hospitalized with extensive burns formed the basis of the present study. On admiission, blisterfluid and serum were collected from each patient and subsequently serum only obtained on 3rd, 7th, 14th, 21st, and 28th day of hospital stay. Complement components C1q, C3, C4, and C5 were measured in these samples by single radial immunodiffusion technique. It was observed that immediately after the thermal trauma there was a sudden and profound fall in serum levels of the early and late complement components vis-à-vis their appearance in blisterfluid. However, their levels in the latter were significantly lower than in the former. The mean serum levels of these components gradually increased up to the 14th day after burns and thenceforth leveled off, although normal serum levels for C1q and C3 were never achieved. Three patients died between 6 and 24 days after trauma. Individual variations in the complement profile were observed and no correlation between the serum levels of complement components and prognosis of burned patients was seen.

Fifty-one patients with penetrating wounds of the thorax at or below the 4th intercostal space were evaluated with peritoneal aspiration and lavage. Nineteen gunshot wounds and 32 stab wounds were studied and there was a 22% incidence of diaphragmatic penetration and/or intra-abdominal injury. There were ten true positive and one false positive study. There were 39 true negative and one false negative lavage. Peritoneal aspiration and lavage is a test of high sensitivity (91%) and specificity (98%). It has proven to be a useful screening technique in patients with penetrating wounds of the thorax. Peritoneal aspiration and lavage identify the patients with diaphragmatic penetration and intra-abdominal injury and minimize negative exploration.

Breast augmentation is being done increasingly, not only for women who consider their breasts too small but also for those with breast asymmetry, and post-mastectomy patients with reconstruction. It appears inevitable that traumatologists will have to evaluate injury to an augmented breast in a patient who has sustained blunt chest trauma. This paper discusses the differential diagnosis and treatment of implant rupture, hematoma, and spherical capsular contracture as a result of trauma.

Three cases of false aneurysms of the superficial temporal artery are reported. Most of these injuries are caused by trauma, frequently sports injuries, more rarely congenital or arteriosclerotic. Diagnosis is made by observation of a mass, usually pulsatile, in the distribution of this artery. Patients who present with a history of injury and a mass over the course of the superficial temporal artery should be suspected of having false aneurysm of the superficial temporal artery. Surgery is indicated to reduce the risk of hemorrhage from subsequent head trauma, to relieve headaches, and for any cosmetic defects.

A case of stable blunt renal trauma is presented. The patient a 20-year-old pregnant woman, was heparinized and 2 days later developed severe renal bleeding. Her kidney returned to near normal with nonoperative measures. Care should be taken in the decision to heparinize such patients following renal trauma because of the possibility of delayed hemorrhage.

A 20-year-old man injured in a motor vehicular accident sustained a nonpenetrating double blowout injury of the upper thoracic esophagus. A simultaneous membranous rupture of the trachea occurred. Treatment was by operative primary closure. A collective review of blunt upper thoracic esophageal perforations reveals a nearly uniform occurrence of associated tracheal injury.

A 4-year-old boy suffered hyperextension injury with small-bowel obstruction and renal injuries. This unusual constellation of findings is discussed in light of the pathophysiologic mechanisms of the injury. Hyperextension of the spine resulted in rupture of the anterior longitudinal ligament and avulsion of the L1–L2 disc; a loop of bowel was then drawn into the disc space. There also was an internal tear of the renal artery. Two years after uretero-ureterostomy with nephrostomy tube, the child is normotensive with adequate renal function and with normal growth and development.

A case of a rare radiocarpal fracture dislocation in a 17-year-old girl, with persisting loss of radiocarpal joint space following reduction under hematoma block, is described. The wrist joint was exposed, and two osteochondral fragments were rotated 90° and secured with 2.7-mm AO screws. Satisfactory healing followed 3 months postinjury.
