The value of informal out‐patient discussions as a useful, additional method of assessment for both patient and doctor during a busy out‐patient clinic is considered. One hundred patients were interviewed by a research or stoma care sister at their first follow‐up visit after surgery for large bowel cancer in the combined Oncology/Surgical out‐patient clinic of a district general hospital. Particular interest was taken of patients’awareness of diagnosis (85% understood) and operation (93% understood), general well‐being (74% had resumed limited activity), dependency on the primary health care team (13%), family history of bowel or associated cancers (as many as 31% had one or more first degree relatives affected) and the suitability for research trials, including adjuvant radiotherapy/chemotherapy (29% were entered). Patients appreciated the opportunity to discuss problems at a particularly stressful period of their lives. It would seem that when problems are identified and an overall assessment of patients’needs made, relevant information is provided for doctors which is influential in improving

Performance status scores are useful tools in the management of patients with malignant disease. No specific performance index score exist, however, for patients with glioma. The Barthel Index, has been used in our department because it has been widely used in patients following stroke and found useful. This study aims to establish whether a modification of the Barthel Index would be more appropriate for patients with glioma and, if so, what modifications should be made.The opinions of multidisciplinary health workers and carers experienced in helping patients with glioma were established by questionnaire. Difficulty with speech and the presence of seizures were statistically ranked the most significant disability in terms of overall performance status. As these are not included in the Barthel Index a modification is justified. However, to include new categories in a performance index but avoid increasing its complexity, existing categories have to be omitted. Therefore, the second aim of this study was to establish which categories within the Barthel contribute least to a change in the total score. Three‐hundred‐and‐thirty‐three prospectively recorded Barthel scores were evaluated in 81 patients over 2 years and the individual categories evaluated by multifactor analysis. The sensitivity of the categories‘bowel function’and‘independence in grooming’to a change in the total score was less than 1%.In conclusion, substituting 'speech difficulties’and the presence of seizure in place of‘bowel function’and‘independence in grooming’from the Barthel Index will improve the specificity for patients with glioma without reducing the sensitivity or acceptability for the patients and research nurses who most often complete them. Over the last decade in the United Kingdom there have been changes in the delivery of health care, technical advances in medicine, and rising public expectations for health care, all situations which effect nurses caring for cancer patients. Nursing cancer patients has frequently been described as stressful. In order to limit stress, it is necessary to monitor nurses’perceptions of its causes. This paper focuses on three studies which have identified cancer nurses’stressors between 1986 and 1993. Although no firm conclusions can be drawn from these results there is evidence to suggest sources of stress have changed. Work overload, lack of resources and staff shortages now seem to be major concerns for nurses. Conflicts with health‐care workers (in particular doctors) has decreased, possibly due to improvements in pain and symptom control management. Patients’deterioration and death continues to cause nurses concern. Job satisfaction remains high, but it has declined since 1986. Breast cancer is the major cause of female deaths in Western Europe. In the United Kingdom, one woman in 14 is liable to develop this disease. An increasing emphasis on recognizing survival from cancer and remission periods of up to 20 years for breast cancer has important implications for health‐care professionals. Care and support should effectively meet patients’needs to enable them to cope with the impact of a breast cancer diagnosis.This paper provides an overview of selected literature on women's experiences of breast cancer. It places particular emphasis on their perceptions of the information they received about the disease and its treatment, and their perceptions of the support available to them.The overview illustrates the problem of inadequacies in the information and support currently available to women with breast cancer.
