Two recent studies carried out by the Medical Research Council Lung Cancer Working Party have suggested that large fraction radiotherapy to the chest in either 10‐Gy single fraction or 17‐Gy two‐fraction doses, 1 week apart, is safe and effective for patients who require palliation for bronchogenic cancer.The Beatson Oncology Centre, Glasgow, participated in the original MRC trial and anecdotal reports of acute chest pains, fevers, sweats and rigors in some patients during the first 24‐hour period after radiotherapy treatment were noted. These acute side‐effects were not monitored during the Medical Research Council trials. It was felt that this area warranted further evaluation in order to identify the incidence of such acute side‐effects and to what extent they caused a reduction in the patients’remaining quality of life.A pilot study of 10 patients confirmed the manifestation of the side‐effects reported anecdotally in the MRC trial. It was on this basis that the study was extended, with a further 51 patients being invited to participate over a 4‐month period.The findings indicate a significant incidence of adverse side‐effects in patients receiving large fraction radiotherapy to the chest in either 10‐Gy single fraction or 17‐Gy two‐fraction doses, but that these are transient and do not cause unacceptable disruption to the patient

Research indicates there is confusion in the minds of patients and the public surrounding the issue of ‘informed consent’. This confusion is compounded when patients are asked to participate in a clinical trial.This study aimed to survey, by self‐completed postal questionnaires, the experiences and opinions of 100 randomly selected healthy volunteers not in a clinical trial, 100 healthy volunteers in a clinical trial and 100 breast cancer patients in a clinical trial.A 71% response rate was achieved. The results indicate that there are areas in the current procedure which scored highly and some where improvements could be made. Overall, 72% of respondents indicated they were satisfied with the level of support they are currently receiving.The results also show that healthy volunteers in a clinical trial were better informed about the trial than the corresponding cancer patients. They were given more written information (86% versus 60%, P<0.005), had a better indication of their time commitment (81% versus 62%, P<0.025), were better informed of possible physical discomforts (61% versus 43%, P<0.05), were better informed of possible side‐effects (75% versus 57%, P<0.025) and were more aware of their rights to withdraw from the trial (84% versus 46%, P<0.005).Patients were also asked for their opinions on their requirements for giving informed consent. The overwhelming majority of the women responding to Questionnaire 2 wanted all the information and support outlined. Only two areas scored below 90%: those were concerned with wanting assurance that all information about the patient will be kept confidential (75%) and those wanting information on protection of the patient's privacy (66%). This paper illustrates the evaluation of some of the effects of a 3‐day workshop for principals and trainees in general practice on a number of aspects of palliative care, using pre‐, immediate and late post‐tests. 摘要:
The purpose of this quality assurance project in the University Hospitals in Leuven, Belgium, was to describe, analyse and improve the actual routine practice of chemotherapy administration. The article focuses on analysis and improvement of role performance of nurses. The process of chemotherapy was systematically assessed by means of a retrospective analysis of patient records. This was done for 61 patient records receiving 453 chemotherapy administrations. Five aspects of the role of oncology nurses were described and analysed: data collection for prescription of chemotherapy, communication of instructions, preparation of the cytotoxic treatment, administration of cytotoxics and post‐chemotherapy assessment. Cancer and its treatment is known to have a substantial psychological morbidity and, while adjuvant psychological therapy (APT) is well established, relatively little work has been done to investigate the effectiveness of psychotherapy, particularly cognitive behaviour therapy (CBT), for patients with cancer.This article focuses initially on emotional responses to the diagnosis of cancer, individual vulnerability and the prevalence of psychological problems in patients with cancer. The use of cognitive behaviour therapy is then described, together with key issues in its application and specific problems. The work concludes by considering differences in applying CBT to patients with cancer compared to patients with other physical illnesses and the notion of ‘relapse inoculation
