The special needs of the dying, and in particular of people dying of cancer, have been studies extensively over the past two decades. The contribution to the debate by those working in primary care has been small. In this paper nine guidelines relevant to terminal care at home are described. They were defined and developed by a small group of primary‐care nurses and general parctirioners, being based on their own professional experience of caring for people dying at home with cancer. Interleukin‐2, one of the group of biological modifiers, has been under investigation as an anti‐cancer therapy since the early 1980s. High doses of this substance produced significant remissions in a limited number of patients suffering from renal cell cancer or melanoma. However, the toxicities experienced by these patients were pronounced and severe. In our institute it was decided to investigate wheter a lower dose of interleukin‐2 given over a longer period could produce similar results but with significantly less toxicity. A Phase I study has been performed, and the Phase II study is now on‐going. The scientific aspects of this study are to be published elsewhere. Roy's adaptation model provides a framework for holistic oncology nursing practice. In this article an overview of Roy's model is provided, and the model is applied to a patient with breast cancer for an assessment of behaviour and the stimuli influencing behaviour, and for the planning and evaluation of nursing care. Also, a comprehensive list of nursing diagnoses is categorized according to Roy's four adaptive modes.

Information on the use of syringe drivers is limited. Small studies have been undertaken in hospices and specialist palliative‐care units in the UK but, because of the small number of cases, these have taken a long time to complete. In this study all of the eleven Marie Curie Centres, hospices funded by a national cancer charity in the UK, have been included, with a total of 327 palliative‐care beds, and admissions approaching 4000 per annum. Information on current practice and policy has been collected by interview, and 3013 sets of patient records have been scanned to identify 1276 cases where syringe drivers were used. Data on frequency and duration of use, drugs and drug combinations prescribed, together with limited records of re‐siting, have been analysed. This article describes some of the on‐going hospice and palliative‐care initiatives in Russia and, in particular, focuses on a pilot nursing education programme on palliative care, implemented in Moscow during the summer of 1991.
