Over the last three centuries, blood gas analysis has developed from a physiologist's tool to an essential part of clinical management. It seems possible that the measurement of acid-base and blood gas parameters will take a new direction by the last decade of this century. A new technology is being developed using optical sensors designed to provide continuous intra-arterial monitoring. This article briefly reviews the history of current methods of blood gas analysis and then concentrates on prospects for the future. The review is divided into the principles of operation of the optical sensors and the practical considerations necessary for this technology to be used for in vivo measurements in critically ill patients. Performance data for the new systems are reviewed, together with their advantages and disadvantages compared with currently available bench blood gas analyzers.

In the perioperative period, patients with ischemie heart disease are at increased risk for cardiac morbidity, including myocardial infarction, congestive heart failure, and dysrhythmias, as well as for sudden cardiac death. Myocardial ischemia is a precursor of these events, and because it is typically symptomatically silent, we must rely on technical monitors. It is hoped that treatment upon detection of ischemie events will improve outcome in patients at risk for perioperative cardiac morbidity. This review focuses on methods used to detect perioperative myocardial ischemia and examines methods of reducing ischemia in this high-risk period.

摘要:
For more than 30 years, anesthesiologists have used intraoperative electrocardiographic (ECG) monitoring to enhance the safety of their patients. Initially, ECGs were used to detect dysrhythmias; more recently, ischemie myocardial changes and infarctions have been diagnosed [1]. However, the complexity of the intraoperative tasks required of the anesthesiologist makes it difficult to continuously monitor ECG tracings. Recent developments in ambulatory ECG monitoring that enable detection of silent ischemia, plus the increasing availability of computer-assisted ECG diagnostic programs, have led to the increased use of automated intraoperative ST-segment monitoring.
