AbstractRadical neck dissection remains the keystone to modern head and neck surgery. Like any major operation, it has inherent complications. These complications become greater when the procedure is combined with resection of primary lesions of the upper food and air passages, and when it follows radiation therapy. In this series, 788 patients with head and neck cancer underwent radical neck dissection alone or in combination with resection of a primary lesion, some with and some without irradiation; the overall complication rate was 50.8%. The total rate included 16.2% of the patients having one or more major surgical complications, 33.5% having one or more minor surgical complications, and 28% having one or more medical complications. The complications are reported in detail to allow comparison with other series and to give a true indication of the overall incidence of the risks involved in major head and neck cancer surgery.

AbstractDespite mechanical and technical advances that have been made in fields allied with microsurgery, Carrell and Guthrie's basic concept of small‐blood‐vessel anastomosis has not changed significantly. However, with refined, finer sutures and needles as well as improved jewelers' and ophthalmic surgical instruments, individuals can now readily practice under high magnification. AbstractThere are many different surgical approaches to the pituitary gland. The earliest approach was intracranial. Extracranial methods (transsphenoidal) then became briefly popular but were subsequently dropped in favor of a return to intracranial methods. AbstractSoft‐palate malignancies classically have been managed with radiation therapy as the principal therapeutic modality. AbstractCurrent research problems and approaches to the embryonic development of the head and neck are surveyed and discussed in terms of congenital defects that occur in this region. Of particular significance are new interpretations of the contributions the neural crest cells make to various components of the head and neck, the influence that the brain and notochord exert on the overall morphology of the head region, the tissue interactions involved in the development and differentiation of the pituitary gland (hypophysis), and the significance of vascular perturbations with respect to malformations in the craniofacial region.

AbstractTherapeutic embolization is now used widely for the treatment, either definitive or preoperative, of vascular lesions within areas supplied by the carotid arteries. Its place in the treatment of arteriovenous fistulas, malformations, and vascular tumors fed largely by the external carotid artery is established, although some of the newer embolic materials have not been fully proven. Balloon catheters appear to represent an important method for treating internal carotidcavernous sinus and similar fistulas. Embolization of cerebral lesions is still practiced on a much smaller scale, and questions as to its efficacy and safety have not been resoived.

AbstractA 16‐year‐old‐girl had a six‐month history of sialotorrhea. Lipiodol sialography of the right parotid revealed overflow of Lipiodol into the right external acoustic meatus. Surgical exploration that left the facial nerve intact disclosed a branchiogenic anomaly confirmed at histologic examination—a lesion that extended parallel to the external acoustic meatus—and aplasia of the pars tympanica of the right petrosal bone. It is concluded that the branchiogenic anomaly is a first‐branchial‐cleft anomaly (type I according to Work); the aplasia of the pars tympanica ossis temporalis remains unexplained.
