AbstractTreatment of the regional lymph nodes forms an integral part of the management of any patient with carcinoma of the tongue. It is the most important determinant of survival. Although survival correlates with the size of the primary tumor, it may also be explained by the higher incidence of metastatic nodes in larger lesions. The incidence of contralateral neck nodes is high, and treatment of the contralateral neck, even in smaller carcinomas of the tongue, must be considered. Anterior tongue carcinomas are equally as aggressive as posterior carcinomas of the tongue. AbstractIn a prospective evaluation, two preoperative courses of methotrexate, bleomycin, and cisplatin combined with 2,000 rad/10 fractions (“chemo‐radiotherapy”) yielded 78% response rates in previously untreated advanced head and neck cancer. Similarly staged patients receiving preoperative irradiation of 5,000 rad alone had a 67% response rate. Treatment‐related mortality in the chemo‐radiotherapy patients was equal to that seen with standard combinations of surgery and preoperative or postoperative irradiation. AbstractThe proper management of patients with orbital and periorbital cellulitis represents a distinct challenge to the clinician. A retrospective study is presented of 165 patients with the diagnosis of orbital or periorbital cellulitis admitted to the Children's Hospital of Philadelphia (CHOP) from January 1975 through December 1980. Sinusitis, trauma, local skin inflammations, and otitis were the most common etiologies. Twenty‐three of the patients had orbital cellulitis, all of which were secondary to sinusitis. There was one death in this series. No patients had permanent ocular sequelae related to orbital or periorbital cellulitis. AbstractSince verrucous carcinoma was first identified as a distinct clinicopathologic entity, a great deal of confusion has developed in interpreting the proper criteria for diagnosis. Various authors, therefore, have developed considerably different concepts of the features that make this lesion a unique variant of squamous cell carcinoma. Consequently, there is great confusion in the literature as to the appropriate therapeutic approach, the incidence of recurrence, and the frequency of anaplastic transformation of verrucous carcinoma. The literature was reviewed to identify all cases of verrucous carcinoma of the oral cavity that appear to meet the clinical and histopathologic criteria for this lesion as originally set forth by Ackerman.1The problems encountered in arriving at the diagnosis of verrucous carcinoma are discussed. AbstractVerrucous squamous cell carcinoma is a distinctive clinicopathologic entity. The lesion's biologic activity places it between conventional (nonverrucous) carcinomas and nonautochthonous hyperplasias of squamous epithelium. The diagnosis of the lesion requires full communication and cooperation between surgeon and pathologist. Once the diagnosis is confirmed, the selection of surgery over radiotherapy should be based on the recorded effectiveness of each modality and not on the phenomenon of anaplastic transformation reported to follow radiotherapy.

AbstractRecent advances in surgery of the larynx and trachea have occurred in the general areas of diagnosis, pretreatment nutritional therapy, surgical technique, and reconstruction. In diagnosis the routine use of the flexible fiberoptic laryngoscope in the office and the use of computerized tomography have been valuable. The addition of enteral and parenteral nutrition therapy in selected patients with laryngeal cancer has been an advance. In the area of surgical technique, section of the recurrent nerve for the treatment of abductor laryngeal spastic dysphonia has proved successful. In the area of reconstruction the use of the epiglottic flap for repair of the larynx after near‐total laryngectomy for glottic cancer and the use of the pectoralis major myocutaneous flap for one‐stage pharyngoesophageal reconstruction have been significant advances. AbstractThe clinician must have a high level of suspicion in order to detect ethmoidal sinus disease. Survey plain film radiographic examination may provide information about gross changes in ethmoidal sinus transparency and the sinus walls. Conventional tomographic study will often aid evaluation of the ethmoidal sinus walls which may be obscured by overlying structures on plain films. Computerized tomography adds a new dimension to ethmoidal sinus study. AbstractTwo patients presented with ulcerative lesions of the hypopharynx. One patient developed dysphagia due to a stenotic hypopharynx which followed an ulcerating mucosal lesion. The stenotic site was reconstructed using a subcutaneous pedicle flap. The advantages of this method are the possibility of a one‐stage operation and a good cosmetic result. In our review of the medical literature, we were unable to find other cases with a similar manifestation treated by surgery.
