AbstractThe anatomy, design, and blood supply of the pectoralis major myocutaneous island flap are described. This versatile, nondelayed flap has, for the most part, replaced the usual cutaneous flaps. Clinical examples of its use in head and neck reconstructive surgery are presented.

AbstractThis is the first article in a two‐part series dealing with the effects and manifestations in the oral cavity of radiation therapy of head and neck tumors. In this section, oral mucous membranes, taste buds, edema and trismus, diet, salivary glands, bone, periodontium, teeth, and composition of oral flora are discussed. Dental management of the dentulous patient is then approached; criteria for preradiation extraction are delineated. In the next issue ofHead&Neck Surgery, the final article in this series will discuss preradiation and postradiation extractions and will elaborate on the dental management (fluoride treatments, follow‐up, and restorative care) of the dentulous patient. AbstractRhabdomyosarcoma represents the most common soft‐tissue neoplasm of the head and neck found in children. The tumor is most commonly seen in Caucasian children under the age of 12, usually presenting as a painless mass. Distant metastases are frequently present, especially in regional lymph nodes, lung, and bone marrow. All patients suspected of having this disorder should have a thorough examination of the head and neck, as well as a complete radiographic evaluation of the primary region. Histologic diagnosis should be made as rapidly as possible. Once this has been accomplished, a thorough search for metastatic disease can be undertaken. AbstractAdenoid cystic carcinomas and adenocarcinomas of salivary tissues are more plentiful in minor than in major salivary glands. In the latter, they comprise no more than 10% of tumors. In both sites, however, their clinical course is characterized by local aggressiveness, recurrences, and a relatively torpid and protracted course, eventually leading to death. The clinicopathologic aspects of these two lesions form the major part of this report. AbstractCancer cachexia should no longer be a contraindication to adequate antineoplastic treatment. Current methods of nutritional assessment allow one to identify malnutrition and to follow the nutritional status of the patient throughout the cancer‐management program. Enteral nutritional repletion and maintenance remain the ideal course of action, but the gastrointestinal tract is not always readily available or advisable for use; in such circumstances, intravenous hyperalimentation (IVH) may be indicated. The properly nourished patient better tolerates cancer therapy, experiences fewer complications of malnutrition (e.g., sepsis and poor wound healing), and has a better‐functioning immune system than does his malnourished counterpart. AbstractIf maximal preservation of appearance and function are to be achieved for the maxillectomy patient, preoperative dental impressions should be obtained, and clear communication should be established between the surgeon and the prosthodontist. The immediate surgical splint, which can be used for edentulous as well as dentulous patients, provides numerous advantages for the patient and the surgeon. Surgical procedures that enhance the fit and comfort of a prosthesis include (1) making the mucosal incision 0.5 cm lateral to the bony cut, (2) extracting the first tooth on the resected side before making the bony cut, (3) preserving the premaxillary segment, (4) creating a skin‐graft mucosal‐scar band, (5) removing the entire velum when the surgical procedure requires resection of more than one‐half of the velum, and (6) evaluating the possible removal of the coronoid pr
