The older antidepressants are associated with a number of troublesome adverse effects and are very dangerous in overdose. The newer antidepressants are normally more acceptable to the patient, though they are by no means free from ill effects, which are of different type, and they are much safer in overdose than the older drugs; it may prove more difficult to withdraw some of the newer selective serotonin reuptake inhibitor drugs.Lithium and carbamazepine are used in the prevention of affective episodes. Patients receiving lithium need to have their serum lithium level monitored regularly, although when lithium levels are maintained below 0.8 mmol/1 and dehydration and drugs that reduce lithium clearance are avoided the risk of neurotoxicity is very low. Carbamazepine is generally a safe drug, although leucopenia has been recorded. While blood counts are recommended at weekly intervals in patients first starting this drug, particularly in those with initially low-normal counts.
