AbstractFor more than two decades, community care has been proclaimed the answer to the 'social problem’ of the ageing of the population. Since the publication of the Community Care White Paper (Department of Health 1989), a plethora of discussion and guidance documents have followed. In this paper, critical examination is made of some of the assumptions that appear to underpin policy and debate about community care. It is argued that recent policy and community care reforms may be affected adversely by inherent conceptual contradictions and conflicting understandings and expectations about community care. It is suggested that the reassuring connotations of the phrase ‘community care’ may not be reflected in its reality and that the debate needs to be extended to acknowledge and address some of the underlying dynamics, contradictions and interactions that may have a bearing on the success of the re

AbstractThis paper describes and discusses an exploratory study which highlights the difficulties faced by employees attempting to balance work with caring for an older adult. Methodological issues arising from the study are explored and issues for further research considered. A significant proportion of the elderly population is cared for by those in full‐time employment. This situation will become yet more significant given the current demographic changes and the pressures that carers are increasingly experiencing to remain in employment. A ‘care audit’ profiling the workforce of a large company was undertaken to explore the issues and the effects of caregiving on employment. Over three‐quarters of the 123 employees who responded reported that they either are, have been, or will be, providing care for an adult in the next few years. AbstractThis paper explores whether normalization theory can provide a useful framework against which to assess the relevance of leisure activity as part of day or residential services to people with learning disabilities. Changing attitudes and patterns of leisure are discussed. Four issues in the normalization principle are examined in detail as they concern the tension between fitting in, and making demands on, society as alternative strategies for achieving respect and integration: image, age appropriateness, use of special aids and group activities. Costs and benefits of segregated leisure activities are reviewed. Practical and psychological barriers to integration are also discussed.

AbstractThe incidence of parenting failure among people with learning difficulties is widely presumed to be high although existing research is characterized by a number of methodological shortcomings. Drawing on material from an in‐depth study of people with learning difficulties, this paper shows that the three variables commonly used for assessing parental inadequacy — child care problems within the family, the admission of a child into care and the termination of parental rights — cannot be taken at face value as evidence of lack of competence or parenting failure. AbstractThis paper reviews the nurse practitioner (NP) literature to address a number of topical issues. It considers whether USA NP experiences translate to the UK, and what needs NPs might meet. The author contends that a ‘care gap’ exists, which can be linked to factors such as inner‐city deprivation, technical knowledge deficits and iatrogenesis, chronic illness, failure of prevention and screening, and rising health expectations on the part of the public. NP roles might be developed to fill the care vacuum in these areas. However, progress to date has been hampered by opposition from the nursing profession itself, as well as from doctors. The paper reviews the empirical evidence on the implementation of NP roles, and considers the implications for nurse education. The NP role is highly demanding, and includes prescriptive authority in many instances. To date, the evidence from the USA studies of NPs’ safety and performance is almost unequivocally positive. To educate NPs in the UK to the high standard of their USA counterparts poses a challenge for nurse educators, particularly in pharmacology and pathophysiology.
