AbstractFollowing the publication of the White PaperCaring for People, changes are anticipated in assessment procedures for community care services for elderly people. From the perspective of those involved in the management and provision of community care, this paper examines the relationship between the proposed reforms and practitioners’ knowledge, opinions and practices. Some implications for training and professional practice are suggested.This study is based upon 33 in‐depth interviews across two counties in North Wales with a range of community care professionals (n=40) and seeks to reflect workers’ understandings about new assessment arrangements and their expections, opinions and anxieties about the anticipated reforms.The data suggest that loss of momentum and enthusiasm for reform has followed the dealy in implementation and that cynicism and anxieties about role uncertainty, resource shortages, perceived training needs and budget holding may be undermining generally positive attitudes to the benefits of holistic asses

AbstractEmpirical and conceptual explorations of informal care have concentrated largely on the difficulties and problems of carers to the virtual exclusion of the gratifications and rewards that can be experienced. Whilst not seeking to minimize the undoubted problems which carers face, this paper attempts to redress the balance by providing evidence that caring can be a source of personal satisfaction for many individuals and that such satisfaction can co‐exist with high levels of stress. It begins by highlighting some common conceptual elements from the available literature on the satisfactions of informal carers. Qualitative data from an investigation by the authors are used to illustrate the range of rewards and satisfactions experienced by informal carers. Analysis of the presence of rewards and satisfactions suggest that factors in the social context of care rather than the personal or dependency characteristics of the cared‐for are important concomitants. AbstractThe aim of this paper is to examine how caring for a terminally ill person in the domestic home restricts the lives of their lay carers. Fifty‐nine lay carers of hospice patients were interviewed in the hospice approximately a week after the patient's admission using a structured and primarily closed choice schedule. Respondents were mainly the spouse or other close kin of terminally ill patients admitted to the hospice. Respondents reported that in the month prior to their admission, patients were highly symptomatic and required a great deal of help with activities of daily living. Forty per cent of patients required help. Three‐quarters of the lay carers reported receiving help from one to three other lay carers in looking after the patient. Two thirds of the carers said that either the patient or they themselves, o both, would have benefited from moe help. Ove one thid of carers reported that in the month prior to the patient's hospice admission their own activities had been severely restricted, especially in the areas of visiting friends and relatives and going out to social activities. AbstractIn the past it has not been possible to compare the distribution and workload of two of the main professional groups providing mental health care in the community, community psychiatric nurses (CPNs) and approved social workers (ASWs) because no adequate national data were available. Two recent surveys permit a limited degree of comparison between the characteristics of the two national workforces and the present paper summarizes the comparison which can be made from the two different data sets, in terms of the numbers of staff and rates per head of population, number of clients seen, caseload variations and training. Training for, and provision of, out‐of‐hours services by ASWs are more uniform, but there is much less variation per head of population in the numbers of CPNs than ASWs across the UK. The CPNs are almost certainly working with more people with serious mental illness.
