Post‐mortem examination of 130 subjects demonstrated a relatively high incidence of ulcerated atherosclerotic lesions of the aortic arch and proximal great vessels. The most commonly affected location was the origin of the brachiocephalic artery. Two patients are described in whom retinal embolism probably originated from this source. Ulceration of the brachio cephalic artery and other proximal great vessels may be an important source of ocular and neurological disability, as well as constituting a hazard for arteriographic catheters negotiating these arteries, such as occurs during arch aortography and selective catheterization of the brachiocephalic arter

This paper outlines a technique for peritoneal lavage with Noxythiolin in severe fulminating peritonitis present at laparotomy. Ten selected patients with fœcal and purulent peritonitis were treated by this method, and the mortality and morbidity were reduced. It is the right of every human being to look human. A new approach to the treatment of gross facial deformity, following the work of Dr Paul Tessier of Paris, is described. Some detail is given of five such cases operated on to date at the Royal North Shore Hospital, Sydney.

The consecutive admissions to hospital of 251 patients for hœmatemesis, melœna or both are documented. Duodenal ulcer was the most frequent diagnosis. Recent salicylate ingestion was found in 87% of patients, and 41% said that they took two or more tablets each day. The overall fatality rate was 11.6%, while it was 7.9% in the combined gastric and duodenal ulcer group. There were more deaths amongst those patients who on admission had a hœmoglobin level of less than 8 gm/100 ml, a blood pressure of less than 100 mm Hg, or a pulse rate greater than 100 per minute, or a combination of these; also amongst those aged over 60 years. The average age of the whole group was 59 years, and the average age of those dying was 72. Hœmorrhage was the major cause of death in 50%. Early clinical diagnosis of ischcemia complicating fractures and dislocations is mandatory. In most patients the restoration of the circulation to the ischamic limb is indicated rather than amputation. To obtain the best results, treatment to restore the circulation and skeletal stability must be commenced early.

A sterile prepackaged disposable applicator for metal skin clips has been used in 72 patients with a variety of standard incisions in the neck, thorax, abdomen, and limbs. The applicator has been found to be simple to use after very little experience. Its outstanding advantage is that it furnishes a significantly more rapid method of closing wounds than the use of interrupted skin sutures. There have been no problems with wound healing, and patients have found no significant discomfort either with having the clips in place or with their removal. There are a few minor points of technique to learn in the use of clips and their removal, but these have been rapidly learnt without any problem by the members of our surgical and nursing staffs.

During the last 35 years 114 children were treated in the Royal Children's Hospital for hydatid disease. The disease is commoner in boys and is more common in the lung than in the liver. The incidence of the infestation at the Royal Children's Hospital has fallen, and a comparison is made with its incidence in the rest of Australia. The presenting symptoms are described, and the treatment of cysts in various sites is discussed.

A hepatic hydatid cyst presenting as an extraperltoneal cyst is reported, and the relevant literature is reviewed. The cyst, which had originated in the left lobe of the liver, had burrowed in between the anterior parietal peritoneum and the posterior rectus sheath, and had reached such an enormous size as to become adherent to the whole of the abdominal wall. Such a presentation has not been reported elsewhere for many years.
