Overuse injury (RSI) of the upper limb is a common and distinct clinical entity brought about by hard continuous use of the limb. Pain is felt in the muscles and joint ligaments whose biological tolerance has been exceeded; this tolerance varies individually and must be genetically determined. These structures are normally non‐tender. Physical signs in overuse injury consist of tenderness in these structures. Such signs in the intrinsic muscles of the hand and ligaments of the wrist joint and carpometacarpal joint of the thumb are virtually diagnostic of overuse injur

Eleven cases of small bowel diverticulosis are discussed. Four patients presented with perforation, five with other sympotms attributable to this condition and in two patients diverticulosis was considered an incidental finding. A review of the literature suggests that small bowel diverticulosis may be: (i)present in up to 1.3% of the population; (ii) associated with symptoms in approximately 50% of patients, and (iii) associated with acute surgical complications in 10% of patients. This may be a disorder of intestinal motility associated with colonic diverticulosis and related to other disorders of smooth muscle and myenteric plexus. Small bowel diverticulosis should not be regarded as a rare, incidental and inconsequential finding.

Born in Scotland in 1807 of a well to do family, Farquhar McCrae received his medical training in Edinburgh and Paris. He was commissioned as Assistant Surgeon in the British Army and in 1839 migrated with his family to become a pioneer grazier, businessman and doctor in the settlement at Port Phillip. He later transferred to Sydney where he was one of the first surgeons elected to the staff of Sydney Infirmary in 1845. Farquhar McCrae died in 1850 after a short but eventful career.

Vertical banded gastroplasty is currently closest to the ideal gastric restrictive procedure for the surgical treatment of obesity. A modification of the technique of vertical banded gastroplasty can be successfully used to revise a failed high gastric reduction for morbid obesity.

An extraordinary case of synchronous development of invasive melanoma in the donor and recipient sites of a split skin graft is reported. The histological features suggest transplantation of a Hutchinson's melanotic freckle (lentigo maligna) at operation 10 years previously as the probable cause. The case is particularly interesting in that the invasive melanoma at both sites was at a similar stage despite a considerable difference in exposure to sunlight in the long postoperative period, perhaps discounting this factor as a precipitating cause of melanoma in Hutchinson's melanotic freckle (lentigo maligna melanoma).

The absence of the internal carotid artery (ICA) is a very rare anomaly. Since the first description of the condition in 1787, only 60 cases have been reported in the literature. We report two cases of absence of the left ICA amongst 2195 carotid angiograms performed in the Department of Neuroradiology at the Royal Brisbane Hospital over a period of 4 years. The first patient was suffering from obstructive hydrocephalus: the second from right cerebellopontine angle syndrome. Cerebral angiography by catheterization of the aorta revealed the absence of the left ICA. The collateral circulation to the left cerebral hemisphere was provided by the vertebrobasilar system via a dilated left posterior communicating artery In the first patient, the absence of the ICA was coincidental with other congenital anomalies of the cervico‐occipital junction. In the second patient. the dilated vertebrobasilar system was responsible for compressing the right cranial nerves VII and VI11. A fourth case of pharyngeal injury after an attempt to open a bottle of ‘home brew’ is presented. For the first time the potential barotrauma causing this injury is recognized.
