A study has been made of the value of the augmented histamine test with medical vagotomy in pre‐selecting the most appropriate operation for any individual patient requiring elective surgery for duodenal ulcer. One hundred consecutive patients were studied. The selection procedure altered the otherwise preferred operation of vagotomy and gastro‐jejunostomy in only eight cases—five patients being treated by vagotomy and anirectomy and three by partial gastrectomy. The results are discussed, and it is concluded that the use of these tests as a preoperative selective procedure is unjust

Factors related to the incidence and severity of early post‐cabal symptoms after four standard operations for duodenal ulcer are discussed. These factors, together with the clinical evidence of the present survey of 277 cases over six years, point to the conclusion that the type of operation in itself has little, if any, influence on the incidence and severity of dumping or bilious vomiting. Symptoms referable to primary duodenal diverticula are unusual and complications are rare. The complications which may arise in duodenal diverticula from mechanical causes or from stasis and infection, and which may follow opcrative removal, are discussed with reference to illustrative cases. The management of selected complications is outlined.

A case is presented in which total colectomy with extraperitoneal ileostomy for ulcerative colitis was followed by the postoperative complication of compression of the duodenum by the superior mesenteric vessels.

The case histories of 211 patients with perforated peptic ulcer treated by simple suture between the years 1949 and 1954 have been studied, in an attempt to determine whether more definitive ulcer surgery is justified at the time of perforation. Certain features in these histories have been investigated to determine their prognostic value at the time of perforation, and an attempt has been made to show the indications for definitive surgery. Of the patients who left hospital, approximately 70% have been reviewed 10 to 15 years after simple suture. It is concluded that there is no justification for adopting a policy of definitive surgery in all cases, but certain indications for this form of treatment are suggested.
