A variety of conservative and surgical options are available in the management of para‐oesophageal hiatus hernia. However opinion is divided in regard to the best form of treatment. A series of 71 patients with para‐oesophageal hiatus hernia has been studied, to assess hospital management and outcome after treatment. Case notes of all patients were reviewed, and a questionnaire sent to surviving patients. Of those patients treated surgically with an anatomical repair plus a fundoplication, 19% had recurrence of significant symptoms. In contrast, 55% of patients managed by an anatomical repair alone had recurrence of significant symptoms. Conservative management was undertaken in 29 of the 71 patients and 66% had recurrence of significant symptoms, with 13 proceeding to elective surgery. Nevertheless, there is a place for the conservative management of para‐oesophageal hiatus hernias. 摘要:
A group of patients with carcinoma of the oesophagus and cardia has been studied. Subtotal oesophagectomy without thoracotomy has been performed. The operation is briefly described, the complications and results are discussed and it is concluded that the operation of subtotal oesophagectomy without thoracotomy does have a place in the treatment of selected patients with carcinoma of the oesophagus.

Although hartmann's procedure has been accepted by most as the treatment of choice for divenicular disease complicated by generalized peritonitis, restoration of continuity may be difficult. A restrospective review of 30 patients, having had hartmann's procedure for complicated divenicular disease, was made to assess restoration of continuity and factors which may influence its performance. This revealed zero mortality with a morbidity rate of 31.5% and a reversibility rate of 85% (with two cases still awaiting restoration of continuity).

Joint mobility was assessed in 25 patients who had undergone surgery for complete rectal prolapse and in 25 age‐and sex‐matched control subjects. A significant increase in extensibility of the fifth finger was found in the patients with rectal prolapse. It was further found that there was a progressive decrease in joint mobility with age in both groups.The pathophysiology of rectal prolapse is complex. Factors considered to be important include rectal intussusception associated with the commonly observed lack of rectal fixation within the sacral hollow, with a deep pouch of douglas and weak pelvic floor musculature. The joint hypermobilily demonstrated in these patients suggests an underlying connective tissue abnormality which perhaps contributes to the lack of rectal fixation within the pelvis and to the rectal wall intussuscept
