SummaryIn the past the orthopaedic problems associated with spina bifida have often been disregarded and rehabilitation of these cases considered impractical. This paper describes the problems encountered in over 100 cases treated by the Orthopaedic Department of the Royal Children's Hospital in Melbourne.The cases have been classified into 4 groups depending on the extent of paralysis. The first group have high lesions with paralysis of the trunk and legs. These are largely aproblem of bracing and, except in neglected cases, offer little scope for surgery. The second and largest group are paraplegic but the psoas muscle is active. In these patients a wide range of surgical procedures for hip dislocation, hip contracture and transfer of the psoas tendon to the greater trochanter will often be required.In the fourth group where there is a combination of paralysis below the knee and gluteal paralysis, correction of foot deformities, their retention by tendon transfer and psoas transplant at the hip will all be part of the surgical programme.The final group with minimal or no paralysis rarely presents as an orthopaedic problem.Secondary deformities occurring in the spine, hip and knees often require considerable ingenuity in their correction. While most of the surgical procedures used in this series are well known, minor but important modifications have been found necessary to achieve success. The timing and planning of the rehabilitation programme is perhaps the most difficult task, and this is described in some detail.A great range of standard surgical procedures is available and with suitable modifications can produce successful results. These are described and the timing and selection ofthe best methods discussed.

SummaryA method of cadaver renal grafting is dexribed based on the patch technique of arterial anastomosis of Carrel and Guthrie (1906). It is fell that the method offers advantagesin potential shortening of ischaemic time and in the ease of anastomosis, particularly when multiple arteries are found.

Summary1. Six cases of subclavian artery aneurysms associated with cervical ribs are reported.2. The natural history of progression from a fusiform dilatation to a grossly abnormal vessel liable to massive or mural thrombosis with embolization is described.3. For those cases with thrombosis and significant ischaemia, aneurysmal formation of great size or fragility, containing mural thrombosis with embolization, it is recommended that the segment should be resected and a graft inserted.4. A method of graft insertion without resection of the clavicle is described.

SummaryFifty‐six cases of carcinoma of the gall‐bladder occurring in two Adelaide teaching hospitals over a five‐year period have been reviewed. Eighty‐four per cent. of patients were over the age of 60 years at the time of diagnosis, and the majority of cases occurred in women. Gall‐stones were present in all except one patient in whom this could be verified. The prognosis, as in other series, proved to be very gloomy, and 61 per cent. of all cases were dead within three months of presentation. The three patients who survived longer than eighteen months, had lesions which were diagnosed as carcinoma only when the specimens were opened after operation.The incidence of carcinoma of the gall‐bladder in 0·5 per cent. of consecutiveroutine autopsies and 3·7 per cent. of patients undergoing surgery for gall‐bladder disease, is greater than in most reported series.The question of cholecystectomy for symptomless gall‐stones is discussed, and it is concluded that in otherwise fit patients this should be recommended, not only because of the risk of carcinoma, but also because of the numerous other troubles and hazards to which these patients are exposed.The more radical surgical approach to established disease by means of right hepatic lobectomy must have a very limited place in view of the age and poor general condition of the maj
