Backache is a common condition. Dissections have been made of human cadavers with the aid of a microscope. The posterior primary rami of segmental nerves have been examined. Particular reference is made to the distribution of medial divisions of the nerves. The innervation of the posterior vertebral joints and the posterior aspect of the sacrum is illustrated.

The surgery of intracranial vascular lesions provides special challenges for the anesthetist. The control of intracranial pressure, the prevention of cerebral ædema and the preservation of cerebral perfusion and oxygenation are leading requirements, to which are added measures to improve surgical access and to lower intravascular pressure in order to increase operability. Appropriate techniques and general provisions for meeting these requirements are discussed

An analysis is presented of 1,235 lesions requiring surgical treatment in 1,045 patients drawn from a series of approximately 11,000 admitted to hospital with head injuries in an II‐year period. Subdural hæmatomas were by far the most common lesions, outnumbering extradural hæmatomas by over three to one. The induction of hypopituitarism has been greatly simplified by a technique in which use is made of the Cooper cryoprobe. This is introduced into the pituitary fossa via the sphenoidal sinus with the aid of an image intensifier. The technique and results are described.

Eighteen cases of hæmangioblastoma of the hindbrain are reported, with reference to age and sex incidence, association with von Hippel's angiomatosis retina and Lindau's syndrome, distribution of lesions and clinical presentation. Forty consecutive cases of ruptured anterior communicating artery aneurysms are studied. Thirty of these were operated on. In 29 the aim of surgery was to clip the neck of the aneurysm. Of these patients, 73% returned to normal life, 9% were left with a permanent mild disability, 8% with a moderate disability and 10% died in the postoperative period. Angiographic spasm was not in itself a contraindication to surgery, the clinical state of the patient being the most useful index. In selected cases definitive treatment to occlude the neck of the aneurysm to prevent recurrent hemorrhage can be undertaken with acceptable results, giving a better outlook than the natural history of an untreated anterior communicating artery aneurysm.
