The management of 75 patients with cerebrospinal fluid rhinorrhœa, meningitis and aerocele due to non‐missile injuries of the anterior fossa is reviewed. In all, 78 operations were done with one delayed death. Fifty‐nine patients had cerebrospinal fluid rhinorrhœa, 24 had meningitis as the leading symptoms, and there were 21 with pneumocephalus. Bilateral defects of the anterior fossa were common, and bilateral exploration has become mandatory. Four cases of dermoid cyst of the central nervous system are presented. The literature is reviewed together with the pathology, and the important point is brought forward that a lump in the midline axis posteriorly may represent a dermoid cyst with an intraspinal or intracranial extension. The risk of infection in an untreated cyst leading to scrious and fatal meningitis is stressed.

This paper presents a collaborative survey of 108 instances of serious arterial injury which were seen over the period 1959–68. The commonest cause of injury was iatrogenic. followed by motor vehicle accidents, industrial accidents, and gunshot or knife wounds. A simplified approach to the inferior vena cava has been used at laparotomy to allow partial interruption of the inferior vena cava in four patients. Access is obtained by incision of the root of the small bowel mesentery, and relatively little dissection is needed. The technique is recommended when vena caval interruption is to be performed at the time of a laparotomy.

A series of 16 endarterectomies of the osteum of the profunda femoris artery is presented. Stenosis of the osteum was detected clinically by the presence of a murmur at the groin, either at rest or on exercise. This was confirmed by angiography with oblique views. Although the superficial femoral artery was completely occluded in every case, no reconstruction of this segment was made, surgery being confined to simple relief of the stenosis of the profunda osteum. Rest pain was completely relieved in 12 out of 13 limbs. There was no residual intermittent claudication in eight of 14 limbs, while the claudication distance was improved in the remaining six.

Patients still present with recurrent parotid tumours which have resulted from inadequate surgery at the initial operation. Such recurrences pose problems in treatment which are illustrated by a review of 130 patients (133 operations) operated upon for tumour, 22 of whom presented with recurrent tumour.

Four cases of lobar sequestration are described, which have been managed in this hospital during the period from February, 1962 to April, 1969. All of them occurred among the local Malaysian population. Only one of these four showed anatomical features that were consistent with descriptions of this condition in the available literature. The other three manifested anatomical variations not consistent with descriptions of lobar sequestration of either the intralobar or the extralobar type, and it is because of this divergence that the following small series is published.
