Experiences with Nissen's operation of fundoplication by the abdominal route in the treatment of sliding hiatal hernia with reflux are described.Cadaver studies were carried out, and showed that the fundal cuff encircling the lower arsophagus has a positive valve action preventing reflux.Twenty‐eight fundo plications were performed, and 23 of these were followed for periods ranging from six months to three years. These patients are compared with another group of 25 patients treated by other operations. Fundoplication provided complete control of reflux clinically and radiologically in over 95% of cases, and the results were superior to those of the other operations.The sequela of fundoplication were more acceptable to patients than those of vagotomy and pyloroplasty. It proved an effective and safe addition to the repair of the hiatus, and its use is advocated selectively for patients with severe reflux œsophagitis.Some modifications of the technique of operation are suggest

Tke results of a study of the value of fibreoptic endoscopy in the assessment of 76 patients with malignant infiltration of the stomach are presented. The Olympus gastroscope (model GTF‐A), the LoPresti axophagoscope and the Olympus œsophagoscope (model EF) were used. Of 76 patients with a radiological diagnosis of gastric cancer, 61 were submitted to endoscopy. Endoscopy was contraindicated in 15 patients for a variety of reasons. In 61 patients it was attempted, but in five the procedure failed. Of the 56 patients who had successful endoscopy, a dingnostic accuracy of gastric cancer was achieved in 87°5%. Fibreoptic œsophagoscopes are superior to fibreoptic gastroscopes in the diagnosis of cancer of the upper regions of the stomach. Scintiscans with technetium‐99m sodium pertechnetate, or technetium‐99m EDTA, were carried out on 10 out of 21 patients with subdural hamatoma or hygroma over the period from March to June, 1970. In each case the scan successfully detected the lesion, which was projected as a rim of increased accumulation of radioactivity. The combination of a positive scan and a shift of midline structures opt echoencephalography is highly suggestive of a subdural lesion, and in the individual patient may obviate the need for cerebral arterio‐graphy. The cases of 60 patients who underwent parathyroidectomy are described. Past, present and future aspects of the diagnosis of hyperparathyroidism, are discussed. In several patients in the series renal calculi had been present for over 20 years before the diagnosis was made. The reasons for this delay are considered.

With continuing improvements in cytological, histological and frozen section techniques, the clinician is becoming increasingly dependent upon the histologist for early and accurate diagnosis of malignant lesions. Newer techniques, using direct smears, fluid aspiration and needle and drill biopsy, will sometimes allow accurate diagnosis to be established with minimal handling of or interference with a tumour mass. This probably reduces the risk of tumour dissemination when compared with more traditional biopsy procedures. When, as with most deep tissue masses, more formal incisional or excisional biopsy is required, the individual circumstances should be assessed and certain appropriate precautions should be taken in order to obtain the most accurate information with a minimum of risk. The scope, suitability accuracy and safety of the various available procedures are outlined in this paper.

Post‐traumatic pulmonary insufficiency and non‐mechanical bleeding are important complications of massive battle injuries. It is suggested that these conditions are inter‐related, and a small series of illustrative cases is described. It is further suggested that this syndrome may be important in some cases of death following severe injuries sustained in civilian life. Although treatment of this syndrome is not yet standardized, the management of the cases presented is described, and its rationale is discussed.
