The experience of our institution is presented to support the proposition that formal sensory retraining procedures are well worthwhile in adult patients after median nerve lesions. The retraining programme is described and the results achieved in our series of patients are discussed, with particular emphasis on the roles of the patients' relatives, occupational therapists and physiotherapists.

摘要:
Ten patients with immediate return of sensation after digital nerve repair were tested for sensibility and showed an early improvement followed by deterioration at the third week. Animal experlments to test the electrophysiology of the sural nerves in the rabbit after microrepair failed to show nerve impulse conducted across the anastomosis immediately after repair. Further clinical observations including nerve blocks supported the hypothesis of overlap Innervation as the explanation for immediate return of sensibility.

摘要:
A plea is made for the early recognition of the possible development of painful conditions affecting peripheral nerves, since it is believed that the experience of our institution, which is detailed here, confirms that while rehabilitation programmes are of benefit to the patient, successful therapy depends upon prompt institution of prophylactic measures to prevent sensitization to sympathetic outflow. Patients are encouraged to use their hands, and it is considered important that they would understand that their complaints arise from a genuine organic condition.

Twenty‐four thumb mutilations have been repaired by using either whole toe transfers or portions of the big toe for reconstruction. The hallux has many of the properties uniquely suitable for thumb reconstruction. 摘要:
We present the results of treatment of completely and incompletely severed digits achieved by the Viennese Replantation Team. These results are regarded as comparable with other internationally reported series. Our extended use of vein and nerve grafts has effected an improvement in functional results and has made possible a more radical approach to severe hand injuries. We have also used composite and compound grafts for fingertip restoration, and utilized joint transplantation or transposition when necessary.

摘要:
Great functional difficulty and mental agony are experienced when there is traumatic amputation of the thumb or multiple fingers or a congenital defect of the thumb or fingers. With the development of microsurgery during recent years, it has become possible to transfer toes to the hand, which is an excellent procedure for reconstruction of defective thumbs and fingers. This procedure was used on 32 patients involving 38 digits with satisfactory results.

Twenty‐five cases of second‐toe transplantation to replace the thumb and other fingers are reported. There were 20 total successes, four partial successes and one total failure. A total of eight patients had circulatory complications at some stage after operation. The causes of the complications are discussed and the preventive measures suggested.
