摘要:
Gastroesophageal reflux (GER), which is associated with respiratory problems such as choking, aspiration, apnea, and chronic respiratory disorders, is a source of significant concern to parents and health care providers. An understanding of the physiologic basis for the association between GER and these disorders enables the nurse practitioner (NP) to act as case manager for these patients. Knowledge of diagnostic procedures and components of management such as positional therapy, feeding techniques, pharmacologic therapy, home monitoring, and social support strategies enables the NP to collaborate effectively with other members of the health care team in providing nonsurgical treatment for these infants.

Job satisfaction influences employee retention, worker productivity, and performance quality. To retain qualified nurse practitioners (NPs), health administrators must identify sources of job satisfaction and dissatisfaction. Herzberg's dual‐factor theory of job satisfaction addresses extrinsic and intrinsic work‐related factors. Expansion of the model to include global job satisfaction and individual differences provided a broad framework for the assessment of nurse‐practitioner job satisfaction. The expanded model was used to analyze the job satisfaction of 132 NPs registered with the South Carolina State Board of Nursing in 1988 (final response rate= 90%). Participants completed the Index of Job Satisfaction (US), the Minnesota Satisfaction Questionnaire‐Short Form (MSQ‐SF), and the Personal and Work Background Questionnaire (PWBQ). Although the NPs were moderately satisfied with their overall jobs, extrinsic factors were found to be major sources of dissatisfaction. The optimal combination of variables predicted by regression analysis to influence global job satisfaction were age, number of children, urban locations, achievement, company policies and practices, creativity, independence, and compensation. 摘要:
Epidural analgesia is an important intervention for both acute and chronic pain management. It has been in use since the early 1900s, but the technique using local application of opiate analgesics has only been in use since the late 1970s (Moulin&Coyle, 1986). Today, many patients receive epidural analgesia for postoperative pain control, and its use for acute or chronic pain management in a hospital, pain clinic, or home setting also continues to increase. Epidural analgesia is also being utilized to manage acute pain in the pediatric client.Epidural analgesia requires meticulous techniques, beginning with placement of the epidural catheter and continuing with administration of medications and nursing management of the catheter. Nursing assessment and development of protocols along with preoperative and postoperative patient and family teaching are vital components of the total plan of care. The nurse practitioner (NP) or other health care provider must be cognizant of safety considerations, whether in the hospital environment, pain clinic, or home setting.

This descriptive study was an investigation of the intentions of 65 senior baccalaureate nursing students to become nurse practitioners (NPs). Students were given a Nurse Practitioner Inventory, which collected information on nine categories related to factors influencing specialization. The data show that a majority of nursing students do not have specific plans to become NPs, although most intend to further their education. The results suggest that this is in part related to a lack of exposure to and familiarity with the NP role.

摘要:
Noninsulin dependent diabetes (type II diabetes) is a chronic disease characterized by hyperglycemia. Clients can generally be controlled by diet and exercise or a combination of diet, exercise, and oral hypoglycemic agents. When this therapy is not effective in controlling the hyperglycemia, the health care provider must choose to initiate insulin therapy. Outpatient initiation of insulin is an alternative to hospitilization in the type II diabetic client. Clinical guidelines for the initiation of insulin on an outpatient basis and management of the diabetic client are presented.
