摘要:
Clinical and community medicine are applied sciences. The medical school is responsible for ensuring that its graduating doctors can apply scientific principles to health problems. That responsibility requires: 1. the selection of subject matter necessary for comprehending those health problems, 2. the presentation of subject matter within its application to explanation of those health problems, and 3. the examination of students through their ability to manage the health problems. If these three requirements are met, medical schools will have established a clear and logical relationship between the means used within the course and the achievement of the ends to which they aspire.

摘要:
Definitions of specific medical competencies to be acquired by medical students in the course of their studies may be derived from the juxtaposition of a list of generic competencies with a list of appropriate subject areas. The end-point of the process is the enumeration and description of specific competencies which may be used for the purposes of curriculum planning and evaluation. The operation of the method is demonstrated in relation to the curriculum of the Beer Sheva Medical School, Israel, whereby a description of a specific competency (‘diagnosis’) is derived by juxtaposing a generic competency (‘problem-solving’) with a subject area (‘clinical practice’).

摘要:
Curriculum changes have become a common occurrence in medical schools. Initial planning of innovative schools have involved clear identification of the philosophy and goals by competent groups of medical educators. The implementation of such curricula usually depends on teachers from various professional specialities at different locations with little or no idea about the curricular strategy. This article identifies the potential use of curriculum analysis for internal refinement of programmes. A scheme for analysis is also discussed as a frame which could be modified to suit the nature of the medical curriculum.

摘要:
Since the integration of professional (clinical) skills and basic sciences at the commencement of undergraduate medical training is still an innovative approach to medical education, the satisfaction of first and also third year students from such a course was evaluated. First year students were satisfied with this approach and demonstrated an ability to initiate by themselves an average of 1.6 hours of patient contact per week to further develop their professional skills. While third year students were able to increase this form of patient contact by 75%, they were less positive in their attitude towards their programme. Also, they had not demonstrated a significant improvement in their ability to deal with problems in this area. However, all students demonstrated an ability to discuss such problems with their peers. Differing expectations may account for some of the differences between the two student cohorts.

摘要:
This article describes the development of an OSCE programme in the problem-based curriculum of the Faculty of Medicine in Maastricht. In each curriculum year students take a skills exam which is based on a sample of the skills taught in the programme. Structured skills training prior to taking an OSCE is advocated as an important tool to overcome problems in defining standards of performance for these exams. The principles of a structured programme are outlined and practical guidelines for the organisation of skills exams for large numbers of students are formulated. Reliability data on the tests used in Maastricht are summarised.

摘要:
This article has traced the origins and changes that have occurred in the Dartford Vocational Training Scheme between the years 1971-1982. It has attempted to assess the effectiveness of the scheme, by asking past trainees for their views on their training and their present status, and by commenting on the results of the more objective measurement of educational attainment within the scheme carried out since 1975 at the Department of General Practice of Manchester University. Some suggestions have been made that may have wider application to vocational training in the United Kingdom as a whole, both now and in the future.
