A technique of mandibular condylectomy and menisectomy is described. Unilateral condylectomy and menisectomy were performed in two horses, and two horses were treated by bilateral condylectomy and unilateral meniscectomy. Minor temporary alterations in mastication occurred following unilateral condylectomy; more severe and longer lasting abnormalities of prehension and mastication occurred following bilateral condylectomy. Malocclusion occurred in all horses following surgery but was only clinically significant in one horse that developed occlusion of the lower incisor teeth 3 mm caudal to the upper incisor teeth following bilateral condylectomy. Maximal mouth opening was normal following unilateral condylectomy and increased following bilateral condylectomy, both immediately following surgery and after 16 weeks. Maximal lateral deviation of the mandible was increased immediately following unilateral and bilateral condylectomy; 16 weeks following surgery one joint had increased, and two had normal and three had decreased maximal lateral deviations. On radiographic, gross, and histological evaluation, the condylectomy sites underwent remodelling and bony proliferation to produce “pseudo” condyles. At necropsy, 16 weeks following surgery, five of the six operated temporomandibular joints contained fibrous tissue with single or multiple cavities lined by synoviocytes. The nonoperated temporomandibular joints were normal. Segments of jejunum that had been subjected to ischemia 1 month previously were resecteden blocin six horses. Everted end‐to‐end anastomoses were performed by application of double rows of stainless steel staples. The anastomotic sites were examined at 3 weeks, 3 months, 6 months, and 8 months postoperatively. All but one of the horses experienced colic at some point after the second postoperative week. At 3 weeks, there was extensive adhesion and stricture formation associated with the anastomoses. With increasing time postoperatively, there were progressively less severe adhesions and strictures. The equine carpal joint was used to evaluate arthroscopic diagnosis of lesions created in joints obtained from horses euthanized for reasons other than lameness. Full‐thickness articular defects were made in 13 sites within the antebrachiocarpal joint and middle carpal joint approximating those found in diseased carpal joints. Arthroscopic evaluation of the lesions included location, depth, and size of the defects. The joints were subsequently examined grossly. Results showed that, when compared to gross evaluation, arthroscopy is capable of accurately identifying subtle changes in articular cartilage and bone. A statistically significant increase in error rate was found for lesions at the medial aspect of the proximal radial carpal bone. Other sites with limited arthroscopic access were the proximal ulnar carpal bone, the proximal fourth carpal bone, and the distal intermediate carpal bone. Arthroscopic approaches to the femoropatellar joint were developed to determine their usefulness for evaluation and surgical treatment of osteochondritis dissecans. It was found that the articular cartilage of the lateral trochlear ridge, medial trochlear ridge, intertrochlear groove, patella, and the lateral and medial reflections of the joint capsule could be examined from an infrapatellar arthroscopic portal. The suprapatellar pouch could be examined partially. Lateral and medial instrument portals were evaluated to determine the accessibility of the lateral and medial trochlear ridges of the femur in the areas where osteochondritis dissecans lesions frequently occur. Sliding the arthroscope sleeve beneath the patella when entering the joint was associated with iatrogenic cartilage lesions. A new technique that directed the arthroscope lateral to the lateral trochlear ridge eliminated iatrogenic cartilage damage.

Forty limbs with femoropatellar osteochondritis dissecans in 24 horses were treated with arthroscopic surgery. Lesions were bilateral in 16 horses and unilateral in eight horses. Diagnostic examination and surgical treatment were performed through a single arthroscopic portal; five different instrument portal locations and six instrument approaches were used. Lesions were localized to the lateral trochlear ridge of the femur in 31 affected joints, medial trochlear ridge in two joints, lateral and medial trochlear ridges together in two joints, lateral trochlear ridge plus patella in four joints, and patella alone in one joint. The lesions consisted of subchondral defects containing chondral or osteochondral flaps or fragments, or were seen as dimpling, cracking, fibrillation, or erosion of articular cartilage, or intact cartilage over a subchondral defect. Loose bodies were found in three joints. There was a poor correlation between radiologic and arthroscopic findings. Surgical manipulations included removal of flaps, fragments, and undermined articular cartilage, and debridement of the subchondral defect. Three horses were euthanized: one electively to assess the joint grossly, one because of complications following surgery and salmonellosis, and one because of unrelated forelimb abnormalities. Immediate clinical improvement after surgery was seen in the 22 horses permitted to survive. Long‐term follow‐up on seven of 10 racehorses revealed that two have raced successfully, two are “ready to race,” three are training sound, two are sound at pasture (still in convalescence), and one has been reoperated. Lateral fenestrations of intervertebral disks T12–13 through L2–3 were performed on 16 1‐year‐old chondrodystrophoid dogs. The disks of five dogs were fenestrated by removing the core of disk material with a 20 gauge spinal needle, six were fenestrated in the same manner with a 14 gauge spinal needle, and five were fenestrated with a 14 gauge spinal needle and a tartar scraper. Forty‐two of 48 disks had radiographic evidence of narrowing following surgery. Microscopic examination of the disks indicated that the nucleus pulposus was nonreactive tissue and that fenestration did not incite an inflammatory response that resulted in dissolution of the nucleus pulposus. Results of histochemical analyses of the fenestrated disks were not different from those of control disks. The only effects observed during a 24 week period following fenestration were disruption of the annulus fibrosus and nucleus pulposus by the fenestration device, removal of varying amounts of the nucleus pulposus, and vascular and fibrous tissue invasion of the lateral annulus fibrosus.
