ABSTRACTThe birth certificate is not only an official, legal document, but also provides critical health planning and research data. This article provides an overview of birth registration in the United States, discusses the functions of the National Center for Health Statistics (NCHS), delineates the role of the CNM in birth certification, presents information regarding the 1989 Revised Standard Birth Certificate, and examines the quality and uses of birth certificate data. The implications of this information about vital statistics for nurse‐midwifery practice, research and education are discussed and specific suggestions are made for improving the accuracy of birth registration data provided by CNM

ABSTRACTThere are over 20 sexually transmissible diseases. This article addresses 10 of them that can affect the health of a pregnant woman, her child, and the perinatal outcome. These are: AIDS, chlamydia, condyloma, cytomegalovirus, gonorrhea, hepatitis B, herpes, mycoplasma, group B strep, and syphilis.

ABSTRACTA prospective study of 1300 low risk patients was completed in a birth center to determine the safety of change in screening criteria for gestational diabetes. The incidence of diagnosed gestational diabetes was 2.8% compared to 5% before the change. Of the 36 gestational diabetic patients, 12 (33%), had a normal glucose screen at 28 weeks but were identified after the 32 week screen. Nine (25%) of the diagnosed gestational diabetic patients were under 24 years of age. The incidence of macrosomia was 7.3%, a decrease from 9.5% before implementing change in the screening process. No hypoglycemia was encountered in the babies born at the center. The results of the study indicate that the new screening criteria is safe for the out‐of‐hospital setting providing that all patients are screened for gestational diabe

ABSTRACTFifty‐nine birthing center clients at term with premature rupture of membranes (PROM) for more than 24 hours were managed on an expectant protocol to see if hospitalization could be avoided without increasing the risk of maternal or neonatal infection. Clients were carefully chosen and were closely followed on a daily basis as out‐patients to assure continued maternal and fetal well‐being. Digital exams were prohibited. Fifty‐four percent delivered without problem in the birthing center and 80% of the total group delivered vaginally. There were five cases of suspected chorioamnionitis and four cases of postpartum endometritis. There were no cases of culture documented neonatal sepsis.
