ABSTRACTAmniotomy is done frequently for the purpose of shortening labor or applying an internal fetal monitor electrode. To determine if amniotomy is warranted as a routine procedure, a systematic review of literature on the subject was done. Studies pertaining to the length of labor showed either no consistent effect of amniotomy, or a shorter labor by a maximum of 61.5 min with early amniotomy. Studies pertaining to the condition of the fetus showed increased incidence of caput succedaneum, skull bone disalignment, and early decelerations with early amniotomy. A small controlled study showed decreased pH of newborns with early amniotomy. There was no significant difference in Apgar score or neurological examination of the newborn with early amniotomy. 摘要:
ABSTRACTA contemporary theme in maternity care is the provision of alternatives for childbirth. This paper focuses on alternative positions during the first stage of labor. The variation in positions used through history are examined. Empirical observations of the influence of positions on labor and various physiological considerations are also discussed. The paper includes recent research findings of the effect of specific positions on labor. Maternal position is viewed as a feature in the conduct of labor that must be considered within the context of a variety of factors influencing both a safe and satisfying labor outcome.

ABSTRACTWith more and more women of childbearing age in the work force, primary care providers must pay attention to the hazards of the workplace, and potential deleterious effects on reproduction. Informed sources point to environmental and workplace exposures as possible causes of birth defects, low birth weight babies, prematurity, mental retardation, and behavioral problems. The intention of this paper is to acquaint the provider with known and suspected hazards for pregnant women and the fetus, as well as discussing exposures to men that may affect their ability to reproduce normally.
