摘要:
ABSTRACTHerpes simplex virus (HSV) infections present most frequently as lesions on the lips or genital area. Specific protocols have been established regarding their management during pregnancy and the postpartum period in order to minimize the risk of transmission to the neonate. However, a significant number of women shed HSV asymptomatically from the cervix; This creates a challenge in obstetric management.Recent evidence shows that antepartum cervical cultures are not predictive of intrapartum cervical shedding. Therefore, any other clues that might help in minimizing neonatal risk are vital. The presence of nongenital infection may indicate an increased occurrence of simultaneous cervical shedding. In fact, there have been cases of intrapartally‐acquired neonatal infection with a maternal history of only nongenital infection. This paper discusses nongenital herpetic infection, including the etiology, and its relationship to cervical shedding. Included is a discussion of previous cases of neonatal infection in mothers with a history of nongenital infection, and five case presentations of buttock infection

摘要:
ABSTRACTThe minor discomforts of pregnancy present difficulties for the health care provider as well as for the pregnant woman herself. Management of the various symptoms requires astute observations and the ability to individualize therapy. Knowledge of a variety of treatment options, therefore, allows practitioners to collaborate with their patients in selecting the best therapeutic approach for the specific situation. This three‐part series reviews the common discomforts associated with upper respiratory infections, minor pain, and gastrointestinal problems as they are manifested during pregnancy. Both nonpharmaceutical and pharmaceutical therapies are discussed. Frequent references are made to the Food and Drug Administration (FDA) Risk Factors which categorize both over‐the‐counter and prescribed drugs according to their documented safety for ingestion during pregnancy.Part I, “Managing Upper Respiratory Infections in Pregnancy,” appears in this issue (JNM 32:6); Part II, “Managing Minor Pain in Pregnancy,” will appear in JNM 33:1; and Part III, “Managing Gastrointestinal Problems in Pregnancy,” will a

摘要:
ABSTRACTSecond trimester mean arterial pressure (MAP‐2) has been studied as a means to predict potential for development of pregnancy‐induced hypertension (PIH). Parameters of predictability have been suggested, tested, and found valuable in adult populations. Although pregnant adolescents are acknowledged to be at greatest risk for PIH, standards have been neither proposed nor tested in this group. This study was developed as follow‐up to a 1985 case study analysis by Novak, Roberts, and Holm19which compares MAP‐2 values of normotensive and hypertensive adolescents. Using retrospective chart review, data were gathered from 84 nulliparous adolescents, 27 of whom developed PIH and 57 who did not. MAP‐2 values proved significantly higher for the group with PIH (M= 82.2) than for those who remained normotensive (M= 71.3) (t(82) = 5.74,p= .000), supporting the research hypothesis that adolescent nulliparas who develop PIH have higher average MAP‐2 values than adolescent nulliparas who remain normotensive throughout their pregnancies. Additionally, MAP values of normotensive adolescents were found to be lower than previously establishe
