ABSTRACTThis study sought to identify some important characteristics of women who reject hospitals as a place of birth and to determine whether those characteristics are also associated with choice between the two nontraditional alternatives available, ie, birth in a free‐standing birth center and birth at home. Data were obtained from 200 mothers on demographics and health‐promoting behaviors, with particular focus on exposure to and information about drugs during pregnancy and childbirth, perceived locus of control, factors entering into the decision to choose one or the other nontraditional birthplaces, and extent of satisfaction with this choice. There were differences between groups in the use of drugs for health‐related complaints, in information about drugs, and in consumption of drugs (including drugs implicated as teratogenic) during pregnancy and childbirth. ABSTRACTThe data from 847 births from a home birth practice where six different positions were used for delivery permitted the analysis of the effect of maternal position on perineal outcome. The obstetrical log also permitted the identification of other factors that influenced maternal position and perineal outcome. The most frequently used birth position was semisitting, in 83% of the births. The incidence of episiotomy was 7%, and of lacerations, 55%. Factors significantly associated with maternal position were fetal presentation (breech) and birth attendant. Fetal position or presentation, infant weight, parity, and the birth attendant were significantly associated with perineal outcome. The predominant use of the semisitting position may explain why there was no association between maternal position and perineal outcome. The influence of the birth attendant on both these factors suggests the impact of attitudes, skill, or ease in assisting the birth on these aspects of obstetrical practice.

ABSTRACTThe Women's Health Care Training Project is a midwifery education program designed for Family Nurse Practitioners and Primary Care Physician Assistants employed in publicly funded clinics in medically underserved areas of California. Developed through the collaborative effort of faculty of the Stanford university Medical Center‐Foothill College Primary Care Associate Program and program consultants from the California State Department of Health Services, the program design permits the primary care practitioner to remain in her community during the student experience, traveling to Stanford for intensive didactic experiences and using local preceptor resources for clinical experience. This decentralized work‐study model of education, which builds on the practitioner's skills in family medicine, expands the accessibility of additional midwifery education to students committed to practice settings in underserved areas. ABSTRACTDue to a worldwide epidemic of sexually transmitted diseases, oculogenital diseases have become an important health issue with respect to incidence, range of syndromes, and severity of symptoms produced. This paper addresses the problem of neonatal conjunctivitis due toNeisseria gonorrhoeaandChlamydia trachomatis. A brief historical overview, the epidemiology, and treatment of ophthalmia neonatorum are presented along with the clinical signs and controversy of present treatment regimes. The two serotypes of chlamydial ocular infections are presented: paratrachoma and hyperendemic trachoma, along with other syndromes they produce in men, women, and infants. In addition, the concept of using human milk as eye prophylaxis is discussed.
