ABSTRACTTwo hundred fifteen women were questioned at six weeks postpartum to identify differences in infant feeding practices among women who had cesarean and vaginal births. Although the study found more women bottlefeeding at six weeks, as compared with vaginal birth women, this difference was not statistically significant (p>.05). Self‐reported “commitment to breastfeeding,” however, was found to be associated with breastfeeding success, irrespective of birth type. Commitment to breastfeeding was the only identifiable common denominator between cesarean birth women and vaginal birth women. Several additional variables were found to be associated with breastfeeding success among vaginal birth women, exclusively. These included: being well‐educated, married, having prior success breastfeeding, feeling prepared to breastfeed, having one's partner committed to breastfeeding, feeling satisfied with breastfeeding in the hospital, feeding the infant soon after the birth, and having a female infant. Results of this study that suggest variables previously associated with breastfeeding success need to be viewed with caution when dealing with cesarean birt

ABSTRACTObservation of 61 successful lactating mothers showed that while some mothers were able to express their breasts easily, other could not. Open‐ended, interactive interviews on the process and problems of expressing were conducted. Data analysis, using grounded theory methods, revealed that expressing had a different meaning to the two groups of mothers. Those mothers who perceived expressing as mechanical and messy experienced feelings of embarrassment and awkwardness; These feelings interfered with let‐down when expressing, and mothers became total breastfeeders or used mixed feeding without expressing. Mothers who expressed their breasts easily had a relaxed attitude towards expressing; The milk obtained increased their confidence in nursing. ABSTRACTA triage system for antepartum surveillance of postdates pregnancy is studied in a busy, indigent care service with limited resources for in‐hospital delivery. One hundred seventy‐eight pregnancies (13.2% of the total population) were thought to be at least 42 weeks' gestation by clinical criteria, and 128 of these underwent at least one complete biophysical profile within seven days of delivery. We find no statistically significant difference in the incidence of meconium staining, fetal distress in labor, or low Apgar scores when either the high or low scoring groups are compared, or when compared with the 50 patients who delivered without a biophysical evaluation. Specific components of the testing schema, however, namely oligohydramnios and spontaneous decelerations on the nonstress test, are highly associated with adverse perinatal events. Of the total population thought to be postmature clinically, only 60 infants (33.9%) were actually beyond 42 weeks' gestation by Dubowitz scoring. We conclude that a modified biophysical assessment as described is easily performed in a nontertiary care setting where a CNM‐MD team approach is applied and may help to differentiate patients who require expeditious termination of pregnancy in‐hospital from those who can be managed expectantly in the birthing

ABSTRACTThe two‐fold purpose of this controlled study is to evaluate the change in pregnancy‐related knowledge by pre‐ and post education testing and to compare the pregnancy outcomes in two adolescent groups. The control group received routine obstetrical care at city health department clinics. The experimental group received care from an adolescent obstetrical service based in a university hospital. The adolescent service team was coordinated by a nurse‐midwife and included a registered nurse, nutritionist, and social worker. A prenatal educational program of nine classes was developed for the special needs of the adolescents. The improvement in knowledge was significant for the experimental group (p<.05). The experimental group experienced significantly fewer low hematocrits (p<.05 andp<.001), less medication while in labor (p<.05), fewer maternal and fetal complications (bothp<.01), fewer blood transfusions (p<.05), and fewer extra infant hospital days

ABSTRACTFetal movement charting has been shown to be a reliable indicator of fetal well‐being. These charts are currently being used in high‐risk pregnancies, but are infrequently used in low‐risk pregnancies because of concern over possible increased maternal anxiety. This study examined the effect of daily fetal movement charting on maternal anxiety in a low‐risk pregnant population. Thirty‐three women were randomly assigned to an experimental (n= 16) or control group (n= 17). The experimental group kept a Cardiff Fetal Movement Chart. All 33 women completed the State‐Trait Anxiety Inventory at each clinic visit. The first, second, and last test scores were compared between groups using thet‐test.
