ABSTRACTTuberculosis (TB) persists as a serious public health problem in the United States in spite of the fact that effective chemotherapy has been available for over 35 years. Over 20,000 new cases and 1800 deaths still occur each year. The continued occurrence of hundreds of cases in young children is especially alarming, as newborns and children under 3 years of age are at great risk of disseminated disease and its lethal complications. The disease persists in part because many health care providers do not consider it a problem. Nurse‐midwives care for a number of groups at high risk of infection (innercity populations, Indochinese and Latin American immigrants, and American Indians) and can be important in helping to eliminate TB. This article provides information about the transmission, epidemiology, and pathogenesis of the disease, as well as guidelines for screening, diagnosis and treatment of TB in both pregnant and nonpregnant women and in newborn

ABSTRACTThe incidence of Gestational Diabetes Mellitus (GDM) reported in an out‐of‐hospital birth center serving 95% Hispanic women was 10%. A program to comanage the care of GDM clients by certified nurse‐midwives (CNM) and their physician consultants is described. The antepartum, intrapartum, and neonatal characteristics of 119 GDM clients between 1983 and 1985 is discussed. Intrapartum and newborn transfers to the hospital were similar when the GDM group was compared to the total population serviced by the birth center during 1984. There were no neonatal deaths in the GDM group. Macrosomia did oocur in 26% of the babies of GDM mothers and shoulder dystocia was reported in 7.6%. The results suggest that a quality program can be established to care for GDM mothers in an out‐of‐hospital birth center. ABSTRACTNutritional intake during pregnancy is important for maternal well‐being and optimal fetal outcome. The nutritional, caffeine, cigarette and alcohol intake of 308 women registering for their first prenatal visit in an urban clinic was surveyed. Twenty‐four hour dietary recalls were coded, computer analyzed, and compared to the RDA for pregnant women for calories, protein, calcium, phosphorus, iron, vitamins A and C, thiamin, riboflavin and niacin. The nutritional intake was inadequate in calories, calcium, and iron. Forty percent of the sample reported smoking at this point in their pregnancy and the mean number of cigarettes smoked was 10 (one‐half a pack) per day. Thirteen percent of the sample acknowledge regular alcohol consumption and 83% of those consuming alcohol also smoked. ABSTRACTThrough the coordinated efforts of MADRE, a United States based organization of women, and AMNLAE (Asociacion de Mujeres Nicaraguense Luisa Amanda Espinosa), the national women's organization of Nicaragua, a midwifery work exchange project was developed. Midwives from the United States participated in continuing education enabling local Nicaraguan midwives to reduce maternal and infant mortality.
