ABSTRACTBirth plans are based on the individual needs of the couple as they prepare for labor, birth, and the integration of a new member into the family. In this paper, it is recommended that birth plans be developed with the family in their home around 28 weeks' gestation. The plan is then used to guide the participants in a structured birth rehearsal as a yardstick with which to review the childbearing experience and as a framework to evaluate the care provided.

ABSTRACTA self‐administered questionnaire was completed by 35 women who had delivered within the last five years and were self‐identified lesbians when they conceived. The majority of women conceived through donor insemination and used the medical care system to achieve pregnancy. All sought prenatal care within the first 16 weeks, 89% participated in childbirth classes, and 80% breastfed for six months or more. Over half of the women (51%) sought obstetrical care from physicians, while 49% selected midwives. The majority (91%) disclosed their sexual orientation to their provider. Overall the women described their experience with obstetrical care providers as positive. ABSTRACTIdentifying what it is about the way nurse‐midwives care for women that might contribute to healthy outcomes for women and infants requires clear definition of that care process. This article describes our efforts to define the theoretical construct of the nurse‐midwifery care process. ABSTRACTA stratified random sample of 500 ACNM members was surveyed by mail in June, 1987, using a questionnaire designed to describe the effects of the liability insurance crisis on the practice of nurse‐midwifery. A majority of the 286 respondents indicated that the increased cost and decreased coverage of liability insurance is having a direct and negative impact on midwifery practice in terms of: financial burden, increased defensive interventions, increased cost of care, decreased availability of care, restricted practice privileges, and stressed employer/employee relationships. A dramatic increase in insurance premiums has occurred without a comparable increase in income. The increased use of defensive medicine reported by cnms has produced higher costs to the public.
