Models of mental health service delivery in managed care have evolved without considering the needs of ethnic minorities in any systematic manner. Consequently, these new systems may pose additional barriers to access and treatment. In this article, the impact of the health care crisis on mental health service delivery to ethnic minorities in terms of access, cost, and quality of care issues in managed care systems is explored. A quality‐of‐care framework is used for addressing the notion ofcultural competence asacritical dimension of quality of carefor ethnic minority populations. Research in minority mental health and quality of care is integrated in order to explore how various structures, processes, and outcomes in managed care systems (e.g., cost containment structures for controlling the supply and demand of mental health services, utilization management and gatekeeping processes) may impact mental health service delivery to ethnic minorities. Social bias is an issue of concern to both practitioners and clinical researchers. This article considers race and ethnic prejudice as a prominent clinical feature in three psychotherapy cases.Diagnostic and Statistical Manual of Mental Disorders(4th ed.) diagnoses, General Adaptive Functioning ratings, and Minnesota Muttiphasic Personality Inventory scores are considered in terms of the level of patient disturbance and severity of out‐group prejudice. Two cases exemplify chronic adverse outgroup ideation, reflecting a constellation of traits of personality disturbance, disinhibttion, and adverse behavioral response (e.g., panic, hostility, and/or aggression) to intergroup contact, while one case evidences prejudicial ideation as a transitory, conditioned response to traumatic victimization by a member of a racial outgroup. The concept of expressed emotion, or EE, plays an important role in understanding psychosocial factors in the course of schizophrenia. More recently, the concept of EE has been applied to affectively ill samples, and our review of this research indicates that, overall, depressed patients with a high‐EE relative are 13 times more likely to relapse than those with a low‐EE relative. Next, we consider whether high EE is an indicator of the depressed patient‐relative relationship, characteristics of the depressed person, or (in some cases) an indicator of parental psychopathology. We address issues pertaining to problematic school absenteeism, a pressing social dilemma that is a substantial correlate of behavior disorders in youth and linked to later problems in adulthood. Adequate taxonomic strategies for school refusal behavior have been elusive and controversial, however, contributing to a lack of consensus regarding definition, subtypes, and assessment and treatment strategies for this population. The evolution of classification systems for this population is discussed, with a critique of the literature based on provisions of explicit, reliable, and applicable criteria, discriminant validity, and interface with clinically useful assessment procedures linked to prescriptive treatments. A reconciliation of past taxonomies is proposed to define school refusal behavior and its essential features as well as specific categories, dimensions, and strategies to organize, assess, and treat this heterogeneous population.

The Myth of Repressed Memorywas written as part of an effort to bring to public attention a concern about some therapy practices that are based on repression folklore. The folklore includes the assumptions that we banish traumatic experiences from consciousness, that we can use special techniques to recover these banished memories, and that they can be reliably recovered and must be recovered in order to be cured. In fact, there is no cogent scientific support for this folklore and ample reason to believe that suggestive prolonged searches for hidden memories can be harmful. After developing false memories, innumerable “patients” have torn their families apart, and more than a few innocent people have been sent to prison. Instead of focusing on the potential problem of false memory creation, or the scientific studies of memory, or even the destruction of families, as I did in the book, Pope attacked the character or ethics, occasionally nitpicking at the methods, of people who appear to support my premises. Pope ignores the book's central message that thousands of people are being harmed by recovered memory therapy and instead attempts to discredit personally several professionals who question recovered memory claims. This commentary examines the nature of science and scientific data relevant to recovered memory assumptions and concludes that many assumptions of recovered memory therapy are falsified. But these claims deeply divide the psychological community. Pope's review contributes nothing positive to the important issues involved and, instead, adds to the mistrust and anger surrounding this controversy. The method of alternate hypotheses and the Principle of Charity are suggested as more productive steps to decrease the polarization.
